The Position:

We’re looking for a full time Registered Nurse (RN) Case Manager, located in the Las Vegas and/or Henderson Area, to add to our Team. In addition to performing visits and completing coordination of client care, the RN Case Manager is the point of contact for all disciplines involved with providing care to patients. The RN Case Manager oversees the frequency of visits for the episode. The RN Case Manager consults with the provider and the interdisciplinary team, giving updates regarding the patient’s care. The RN Case Manager ensures the medication profile remains current, ensuring changes in condition have been reported to the physician timely. This position would be best for an RN Case Manager with experience, excellent assessment, and communication skills.

The Responsibilities:

• Provides intervention, ongoing education, and support, as needed related to terminal illness and loss.
• Assesses and addressed symptoms associated with terminal illness.
• Facilitates communication among patient, family and hospice team.
• Provides personal care as needed.
• Works closely with the Patient Care Team.
• Adjusts the visit frequencies, according to the patient’s need.
• Continually collaborates with the physician and interdisciplinary team to develop the plan of care.
• Oversees the coordination of care, in partnership with the care team.

The Requirements:

• License Registered Nurse (Nevada)
• Minimum of 1 year experience as a RN in a hospice/home health setting
• CPR/First Aid certified
• Reliable transportation and maintained vehicle insurance
• Current Nevada Driver’s license
• Able to successfully pass a background and drug screening pursuant to NRS 449.188 and NRS 179A.100

Pay: $72,000.00 - $87,000.00 per year
