We are seeking a Registered Nurse (RN) to:
- Provide evidence based, high quality nursing care for patients via telephone triage and MyChart
- Assess patient care needs, provide patient and family education, and implement a plan for care while on live telephone triage.
- Support providers and patients through team based follow up
- Utilize delegation protocols, practice guidelines, and specialty specific resources to guide triage decision making.
- Work as part of a multidisciplinary team to optimize care coordination.
- Establish goals and strategies for meeting needs across settings with the patient, family, and/or other care providers.
Education :
Minimum – Graduate of school of nursing.
Preferred – Bachelor of Science in Nursing (BSN).
Work Experience :
Minimum – Six (6) months of RN experience.
Preferred – Relevant RN experience.
Licenses and Certifications :
Minimum – Registration as a registered nurse in the state where employed or licensure in a state in the licensure compact and CPR/BLS Certification.
Preferred – Applicable clinical certification.
