Sample Supports is seeking a Residential Team Supervisor who uses independent judgment and discretion to support current host home placements, family caregivers, and those in the supported apartment program, in addition to supervising and training the residential team. This position is responsible for monitoring the quality of services for and protection of placed adults, provides case management for individual adults, coordinates services for adults and their providers with the Division of Developmental Disabilities, and provides monitoring and support to those in residential services. They also provide direct supervision to their team members, develop and lead ongoing training, and assist coordinators with task monitoring and compliance.

Duties and Responsibilities:

  • Supervising residential team members including direct care staff and coordinators
  • Facilitate billing, including generating a weekly billing detail report to review and compare, creating list of discrepancies
  • Resolve billing discrepancies as they emerge, this includes follow up and retraining for providers
  • Follow up on incident reports daily, and after they are edited send to manager for final approval
  • Complete ECP monitoring for caseload, reporting if 3 occur in a 30 day period.
  • Provide ongoing support and training to the team as well as the host home provider
  • Maintaining a supplemental caseload based on the needs of the program
  • Completes unannounced home visits on a yearly basis to evaluate compliance with the residential inspection
  • Works closely with managers and directors in developing clinically appropriate treatment, safety and behavior plans for each adult
  • Provides ongoing training and resources to high-needs clients and families
  • Works closely in coordination with case managers, guardians, advocates, state personnel, therapists and families to monitor the adult in placement
  • Completes quarterly audits of the provider and client files, and ensures that all documentation is in compliance with CO Developmental Disabilities Standards
  • Prepares written documentation for court hearings, administrative reviews, HRC reviews, team meetings, etc.
  • Participation in relevant meetings for each adult on caseload to ensure all needs are being advocated for appropriately and that rights are being protected
  • Attends ongoing clinical supervision with manager to review caseload, discuss treatment plans and any other issues that arise
  • Respond to emergencies involving adults on caseload, and be available “on-call” on a 24/7 basis
  • Advocates for the development of services/supports needed for people with developmental disabilities

Qualifications:

  • B.A/B.S in the social sciences preferred
  • 1-2 years of experience working with adult in substitute care desired
  • 1-2 years of experience working with individuals with special needs and/or disabilities, including those with a history of severe trauma and/or neglect desired
  • Ability to effectively work independently with minimal supervision
  • Effective communication skills both oral and written
  • Ability and willingness to work some hours outside of normal working hours to respond to emergencies or to otherwise meet the needs of individuals with disabilities and their families
  • Valid Colorado driver's license and ability to meet the driving requirements
  • Consistent access to reliable transportation that would allow for quick response in an emergency
  • Must be competent and willing to perform Non-Violent Crisis Intervention physical managements as trained
  • Able to perform the physical demands of the position

Salary:

$50,952 annual salary plus metric based bonus
