JOB DESCRIPTION
JOB TITLE: Registered Nurse
IMMEDIATE SUPERVISOR: Director of Nursing
PURPOSE OF JOB: To assess the nursing needs of aging residents; plan and implement nursing care to meet those needs and evaluate the effectiveness of such care to achieve and maintain a level of wellness consistent with the limitations imposed by the aging process.
RESPONSIBILITIES AND DUTIES:
A. Essential Tasks:
1. Performs the following supervisory duties as assigned (non-inclusive):
a. Supervise general nursing care provided by Nurse Assistants/ Orderlies, Licensed Vocational Nurses and Medication Aides.
b. Make daily shift assignments based on resident needs, residents
census and personnel qualification.
c. Assure that goals and approaches from Resident Care Plan are
conveyed to and implemented by the Nursing Staff.
d. Observe direct care administered by staff ensuring safe, effective
care is given in a timely manner.
e. Observe performance of assigned duties by staff to ensure timely,
efficient completion of all assigned tasks.
f. Supervise and assure completion of daily and weekly documentation in the Resident Medical Record, Resident Care Plan and other required facility forms.
g. Supervise and make recommendations regarding safe and effective feeding practices.
h. Identify training needs of assigned staff, in resident care, and
implement mini in-services.
i. Give oral instructions regarding nursing care, care plan changes,
and end-of-shift report.
j. Initiate, contribute to, and communicate the plan of care for
residents, according to resident needs and policy thus promoting
continuity of care from admission through discharge or transfer.
2. Perform administrative duties as assigned (non-inclusive):
a. Maintain medical records.
b. Prepare Nursing Care plans.
c. Complete Accident and Incident Reports, and Infection Control
Reports according to facility policies and procedures.
3. Render general nursing care to residents.
4. Administer prescribed medications and treatments in accordance with
approved nursing techniques.
5. Observes residents, records significant conditions and reactions.
6. Notifies supervisor or physician of resident’s condition and reaction
to drugs, treatments and significant incidents.
7. Takes temperature, pulse, blood pressure and other vital signs to detect
deviations from normal and determine progress of residents.
8. Change dressings and clean wounds.
9. Explain procedures and treatments to residents.
10. Implement restorative nursing measures, correct aseptic techniques, and
other infection control measures when providing nursing care.
11. Make daily rounds, visiting each resident at least twice to monitor resident condition and responses to treatment.
12. Practice good body mechanics and monitor practices of assigned staff,
encouraging use of available lifting equipment, devices and labor saving
practices.
13. Monitor environmental factors for potential safety hazards.
14. Remove residents from area (s) of potential hazard (s) in accordance with
facility policies and procedures.
15. Comply with and monitor adherence to infection control and medication
policies and procedures.
16. Maintain current knowledge of scope of practice for self and others and
recognize legal implications when making nursing judgements.
17. Ensure that safety precautions are incorporated in the Resident Care Plan
and communicated to staff.
18. Inform the Director of Nursing/Assistant Director of Nursing, attending physician, sponsor or other appropriate staff regarding changes in resident
physical, emotional or behavioral status.
19. Communicate with therapist to ensure follow through with rehabilitative measures.
20. Report defective equipment or potential hazards for immediate repair or
replacement.
21. Maintain a courteous and helpful attitude toward residents, visitors and
staff members.
22. Practice effective personal health and hygiene habits.
23. Maintain uniform and personal appearance standards in accordance with
facility policies and standards.
24. Maintain personal conduct that reflects favorable public relations for the
facility.
25. Perform Licensed Vocational Nurse duties when necessitated by staff
shortages, resident workload, etc.
26. Perform other duties as assigned.
B. Marginal Task:
1. Performs the following supervisory duties as assigned (non-inclusive):
a. Contribute to topics for regular in-service programs.
b. Participate in the staff evaluation process with the Director of Nursing and Assistant Director of Nursing, recognizing superior performance and performance or conduct not in keeping with the facility philosophy or standards to assure retention of competent, qualified staff.
c. Monitor performance of private duty attendants.
d. Ensure facility policies, procedures and standards manual is located in a visible and accessible location for ready reference by staff.
e. Adhere to and monitor efficient and courteous telephone use in
accordance with facility policy.
2. Prepares equipment and aids physician during treatments and examinations of residents and completes documentation of visit.
3. Teach residents (nutritional needs and self-care).
4. Make physician appointments, therapy appointments and arrange for
transportation to appointments.
5. Participate in required fire drills and monitor performance of staff.
6. Maintain a clean, orderly and safe environment for the resident, visitors,
and co-workers.
7. Use, interpret and convey information related to resident care policies and
procedures.
8. Consult with and inform consultant Dietician or Food Service Director
regarding special nutritional needs or changes in diet.
9. Ensure clear identification of staff members and their role to resident, and
facility visitors and promote reality orientation measures and programs.
10. Perform basic nursing duties (Nurse Assistant/Orderly) when necessitated
by staff shortages, resident workload, etc.
C. Shift Specific Tasks: (Nurse Assistant/ Orderly):
1. 7:00 a.m. to 3:00 p.m.
1. Check assignment at Nurses Station.
2. A Nurse Assistant/Orderly from each hall must complete vital signs and have them recorded on assignment sheet no later than 8:00 a.m.
3. Load clean linen carts and obtain soiled linen barrel for each hall.
4. Begin getting resident up, toilet them, wash their hands and face, dress and
groom for the day. No pj’s unless Nurse makes an exception.
5. When breakfast trays arrive, serve trays, making sure resident is sitting up in bed and eyeglasses are on, dentures are in place, silverware is on the tray; open everything on the tray. Make sure the call bell is within residents reach.
6. After trays are passed, all aides feed residents who cannot feed themselves. The floater continues to answer lights and assist with feeding.
7. Pick up all breakfast trays, mark diet sheet appropriately.
8. Proceed with resident care.
a. bather follows bath schedule
b. on non-bath days, resident receives peri-care before being dressed and groomed for the day
c. all residents receive mouth care or are assisted as needed
d. male residents are shaved on bath days or daily as necessary
e. fingernails and toenails should be kept neatly trimmed. If the toenails are a problem, notify the Nurse.
f. apply makeup to female residents as desired.
9. On bath days, resident’s bed receives a complete linen change, however,
change any sheet that is soiled as needed.
10. Make beds and straighten rooms.
11. Take residents to appropriate activities.
LUNCH BREAK 1stand 2nd- 10:45 – 11:15 11:15 - 11:45
12. Make rounds of incontinent residents before lunch changing diapers, taking residents to the bathrooms as needed.
13. Take all residents to the Dining Room by 12 noon. Check with nurse for
exceptions.
14. Put bibs on appropriate residents, serve lunch trays, setting up tray for each resident. Mark diet sheet after meal.
15. Return residents to their rooms, toilet them, change diapers and put to bed for a nap. Leave call bell within residents reach. Side rails up if necessary.
16. Any repeat V.S. or B.P.’s foley out puts, BM’s are to be recorded on
assignment sheet by 2:00 p.m.
AFTERNOON BREAK 2:00 - 2:15 2:15 - 2:30
17. Sort dirty linen barrels.
18. Make sure, kitchen, dining room and break room are clean.
BATHER
1. Bathe 1 -3 residents before breakfast - total baths number from 11 – 13.
2. Clean gurney and tub between baths.
3. Bather does not have to dress resident but may assist with some of the more difficult residents to dress.
4. After each bath, disinfect tub and gurney (lock disinfectant in cabinet in
bathing room), restock supplies in bathing room.
5. Prepares breakfast carts:
Styrofoam cups, trash bag at the end of the cart
Make coffee (decaff – 4 – 5 pots)
Make sure all residents get a tray.
6. Assist with feeding breakfast and lunch; pick up bibs, put in soiled linen
barrel.
7. Assist with putting residents to bed for a nap.
8. Refill soap containers for all bathrooms, break room, utility and exam room.
LUNCH BREAK 1stand 2nd11:00 – 11:30 11:30 – 12:00
1. Answer lights as soon as shift begins, assist with breakfast and feeding
residents.
2. Assist with resident care.
3. Floater is responsible for getting residents to appointments with in-house.
Dentist, Podiatrist and Beauty Shop. These will be indicated on the
assignment sheet.
4. During the lunch hour:
pass trays
answer lights
refill water pitchers with ice
observe residents eating in their rooms
' put clean set of towels for each resident in bathrooms
5. Assist with last rounds, usually help on the men’s hall.
* There is a list of residents who need to be restrained at the Nurse’s desk. Please refer to this or check with the Nurse if there is a question. Make sure all side rails are up (check with Nurse for exceptions) and call bell is within residents reach when you leave.
2. 3:00 p.m. to 11:00 p.m.
1. Check assignment.
2. If no floater is scheduled, each hall must complete vital signs by 5:00 p.m. or earlier.
3. Load clean linen carts and bring one soiled linen barrel for each hall.
4. Check hall for whereabouts of residents.
5. Answer lights and begin getting up residents. Dress properly for dinner. No pajamas. Be sure that faces are washed, perineal area clean, dentures in place, hair combed and glasses on. Tidy up each room before leaving. Take resident to dining room by 4:40 p.m.
6. If time permits, empty linen carts, distribute resident’s clothing to rooms and/or pass towels.
7. DINING ROOM:
Pass all trays before feeding. Whatever tray you pick you pass. Take turns feeding different residents. After feeding, take the residents back to their rooms, and pick up trays from dining room and halls (bibs too). Employees are not permitted to eat food from the resident’s trays.
8. When putting to bed, check restraint list. Do good p.m. care when putting
residents to bed. Brush teeth, clean and soak dentures. Wash their face, pericare and turn residents on their side. Prop with pillow and be sure pads and diapers are under their bottoms and not their backs. Place call light within reach. Raise bed rails.
9. After residents are in bed answer lights, pass any clothes, towels and water
that has not been passed. Check residents every two hours.
10. Empty foleys at 9:30 p.m. Chart BM’s.
11. Last rounds should be done between 10:00 – 10:15.
12. Sort all soiled barrels before leaving.
13. Make sure kitchen, dining room, and break room are clean.
BATHER
1. Check bath list.
2. Make sure bathroom is stocked with all necessary equipment. (Towels, soap shampoo, lotion, razors, shaving cream, soiled linen barrel, etc.)
3. After bath, return resident to room and re-dress. Put call light on if you need assistance.
4. Clean gurney and tub between baths.
5. After all baths are finished, clean bathroom thoroughly. Put towels away,
refill soap bottles. disinfect tub and gurney with spray (lock disinfectant in cabinet in bathing room). Return bath gurney to tub.
6. If there is no floater, pass towels and water.
7. Help answer lights on both halls.
8. Do last rounds on men’s hall.
9. Check to make sure the kitchen, dining room and break room are clean.
FLOATER
1. Take all vital signs. (Daily and weekly) Vitals must be completed no later than 5:00 p.m.
2. Help answer lights on all halls.
3. After dinner, help put 2 -3 residents to bed on the “men’s hall” and then go to the “ladies hall” and put 2 -3 residents to bed. Still help answer lights.
4. Pass ice water and towels.
5. Do Last rounds on “ladies hall”. Make sure kitchen, dining room, and break room are clean.
3. 11:00 p.m. to 7:00 a.m.
1. Listen to Report, check assignments.
2. Put hanging clothes in closets.
3. Take vital signs as indicated on assignment.
4. Obtain soiled linen barrel, make 12:00 midnight rounds of all residents.
5. Perform assigned duties that are appropriated for the day:
MONDAY -- Clean wheelchairs, bed pans and water pitchers.
Room 1 – 6
TUESDAY -- Clean wheelchairs, bed pans and water pitchers.
Room 7 – 12
WEDNESDAY - Clean wheelchairs, bed pans and water pitchers.
Room 13 -18
THURSDAY -- Clean wheelchairs, bed pans and water pitchers.
Room 19 - 24
FRIDAY -- Clean wheelchairs, bed pans and water pitchers.
Room 25 – 30
6. 2:00 a.m. rounds – all residents.
7. Finish assigned duties.
8. 4:00 a.m. rounds – all residents.
9. Take routine BP’s and V.S. as indicated by Nurse.
10. Freshen water pitchers, pass ice.
11. 6:00 a.m. rounds, get up designated residents as indicated by Nurse.
12. At the end of the month assist Nurse with paper work
13. Routine duties:
Make sure bedpans, urinals, wheelchair, water pitchers,
are labeled.
Clean all refrigerators at least weekly.
Change soap solution in Johnny Mop containers at least
once each week and PRN.
Leave break room and utility rooms in good order.
FLOATER
1. Responsible for all V.S. – Answer lights when rounds are being done.
Responsible for incontinent rounds on residents 13 – 30.
