Rinehart Clinic has an immediate opening for a Referral Coordinator. This is a full-time position with Medical, Dental, and Vision benefits effective on the first day of employment. Job Purpose: This position is responsible for coordinating all patient referrals. Reflects Rinehart Clinic’s values: Excellence, Caring, Integrity, Accountability, and Teamwork.

Duties:

  • Maintain ongoing tracking and appropriate documentation of patient referrals within the electronic medical record.

  • Ensure complete and accurate registration, including patient demographic and current insurance information.

  • Assemble information concerning patient's clinical background and referral needs. Per referral guidelines, provide appropriate clinical information to specialist.

  • Contact review organizations and insurance companies to ensure prior approval requirements are met as needed. Present necessary medical information such as history, diagnosis, and prognosis.

  • Review details and expectations about the referral with patients as needed.

  • Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services).

  • Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. May assume advocate role on the patient's behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion.

  • Focus on open referrals. Has patient been scheduled? Are there barriers to scheduling? Update referral status and communicate with team members as appropriate.

  • Manage the clinic’s utilization of community referrals to providers within the Tillamook and Clatsop regions.

  • Work with the clinic’s Community Health Workers to connect patients to the appropriate social, community, and cultural resources as needed.

  • Establish and maintain relationships with identified service providers.

  • Ensure that referrals are addressed in a timely manner per clinic policy and procedures.

  • Remind patients of scheduled appointments via mail or phone.

  • Ensure that patient's medical record is up to date with information from specialist consults, hospitalizations, ER visits and community partners related to their health.

  • Collaborates proactively with all team members and with a patient-focus to facilitate and maximize patient healthcare outcomes.

  • Ability to expand role and duties based on clinic and patient needs.

  • Participate in the clinic’s quality improvement activities.

  • Attend all appropriate meetings.

  • Comply with clinic’s policies and procedures.

  • Perform other related duties as assigned.

Qualifications:

  • High school diploma.

  • Experience in customer service.

  • Good verbal and written communication skills

  • Prior experience in healthcare.

Physical Requirements:

  • Ability to communicate both in person and by phone.

  • Ability to lift up to twenty-five (25) pounds.

  • Ability to sit for long periods of time.

  • Manual dexterity when inputting computer information.

  • Exposure to normal noise, temperature, and dust and fume levels.
