Peak Vista Community Health Centers is a nonprofit health care organization whose mission is to provide exceptional health care to people facing access barriers through clinical programs and education. We provide integrated health care services to include medical, dental and behavioral health throughout 24 outpatient health centers within El Paso, Teller, Lincoln, Adams and Kit Carson counties. Compensation (Pay): $16.25 to $23.56/hourly, based on experience.
Summary of Benefits:
Job Summary: The Referral and Resource Navigator's primary role is to assist identified patients with identifying barriers to effective care, assisting the patient in identifying and working with community resources and is key in obtaining medically appropriate services through referral processing inside and outside of the Peak Vista Community Health Centers. The Referral and Resource Navigator provides one-on-one guidance and support through the healthcare continuum. The Referral and Resource Navigator will utilize all referral processes applicable to the payor source, including pre-certifications and prior authorizations. Resource Navigators collaborate with clinical staff, emergency diversion team, centralized care managers, health education and community providers to ensure smooth transition to the most suitable level of care. Referral and Resource Navigators promote patient education and self-care management in order to reduce hospital readmission rates; through interdisciplinary collaboration with the care coordination team to achieve measurable patient outcomes. In addition to promoting and coordinating healthcare services, with cost effective solutions, the Resource Navigator contributes to the implementation of the member's care plan.
Qualifications:
Education: High School Diploma required. Associated degree preferred.
Work Experience: Minimum 1 year experience in a healthcare or social service setting is required.
Cognitive Skills (Language, Math, Reasoning Ability): Excellent verbal and written skills in English and Spanish preferred.
Computer Skills: Microsoft applications required.
Other Skills: Ability to flourish independently and in a team management system. Sensitivity to low income, ethnic minority community. Excellent organizational skills, excellent tracking and documentation required. Excellent attendance required. Excellent listening skills.
Certificates & Licenses: Valid Colorado driver's license and a dependable and reliable vehicle.
Physical Demands: Frequent standing and walking, occasional long periods of sitting, moderate bending, moderate energy requirements, and frequent fine motor skills. Viewing computer screen continuously for long periods of time, hearing within normal range. Must be able to lift 20 pounds.
