Job Summary:

The CQI Data Coordinator is responsible for supporting quality improvement activities on both the programmatic and organizational levels. These activities include data collection, analysis and preparation for use by the Director of Quality Improvement and CQI Data Analyst.

We are seeking an experienced candidate for our full time (35) hour work week Continuous Quality Improvement Data Coordinator role. The hours are 9:00 a.m. - 5:00 p.m. Monday through Friday and offers a partially remote option.

Duties and Responsibilities:

• Assist with the implementation of the Agency’s Orientation and Personnel Development Program. This includes data entry and reporting.
• Coordinate all aspects of the Elder and Social Services Case Record Review Committee.
• Coordinate data collection related to stakeholder involvement activities including Satisfaction Surveys (Client, Personnel and External Stakeholder) and Outcome Measurement.
• Coordinate and monitor data including but not limited to Risk Management, Utilization Review and Case Record Review findings to be used by the CQI Data Analyst, Director of Quality Improvement and Agency leadership to stimulate future quality improvement activities.
• Assist Director of Quality Improvement to gather data relevant to the Agency’s Quality Management Plan and other Quality Improvement initiatives.
• Assist the Director of Quality Improvement with accreditation (and licensing as appropriate) preparation and implementation activities.
• Conduct site visits for the purpose of data collection including but not limited to Facility and USDA reviews. Reports findings as appropriate to CQI and program leadership.
• Assist with internal program reviews and compliance audits as requested by Leadership and the Director of Quality Improvement.
• Assist with the Worksite Wellness committee activities including scheduling meetings, maintaining minutes, coordinating and organizing events and maintaining a comprehensive record of Worksite Wellness activities.
• Assist Director of Quality Improvement to update and maintain electronic Policy and Procedure manual.
• Perform all other duties and responsibilities held reasonable for this position and at the discretion of the Director of Quality Improvement.

Education and Experience:

• Must have a minimum of a Bachelor’s Degree in related field.
• At least 2 years of experience leading quality improvement activities for an agency or corporation.
• Experience working with JCAHO, CARF, or COA preferred.
• Excellent written and communication skills required.
• Must have demonstrated computer skills to work with MS Office, Excel and Access Databases.
