Position: Program Director- Therapist
Location: Lockport & New Lenox, IL
Schedule: 8:30am-5:00pm M-F (one late night per week 11:30am-8:00pm)

Job Description:
- Responsible for managing all phases of the Community Support team, including the planning, coordination, oversight, and delivery of clinical and supportive services to meet the person’s needs; and the identification, development, and maintenance of relationships with various community-based agencies or resources which can provide new, needed, and complimentary adjunct services for persons served
- Participates in and manages resident intake; collects all necessary clinical documents
- Completes mental health needs assessment and diagnosis, and develops, implements, and reviews and revises each person’s goals and treatment plans in response to the person’s housing, mental health, educational, vocational, and community support needs.
- Completing assessments, such as IATP’s, LOCUS, DLA, etc
- Completes case management tasks for persons served, including but not limited to benefits applications, budgeting, applying for utilities, medication management, and assistance with health needs
- Supports residents during medical and psychiatric appointments
- Helps residents develop independent living skills, such as cooking healthy meals, clean, doing laundry, etc.
- Provides crisis intervention to residents when necessary
- Participates in an on-call rotation with other members of the Community Support Team
- Reviews and provides feedback to staff on submitted Medicaid mental health documentation
- Collaboratively works with the Property Manager to help persons served maintain stable housing
- Complete monthly apartment inspections with Property Manager
- Participates in weekly leadership meetings, collaborating with the Behavioral Health Leadership team
Minimum Qualifications:
- Master’s Degree in social work, psychology, or related field.
- License or working towards licensure is preferred.
Salary:
- Masters Degree: $56,000
- Masters with license: $60,000
- Eligible for up to $1500 quarterly bonus.
- Supervision towards licensure included
- As a non-for-profit organization, employees may be eligible for federal student loan forgiveness programs
