GENERAL SUMMARY:

Provide necessary independent living services to youth 16-24 transitioning into adulthood to assist them with becoming self-reliant & independent. Provide initial assessments, individual and group sessions and basic case management functions, along with performing screening and referrals for mental health and community resource services for program members. Community Navigators will help POWER members eliminate barriers, challenges and disparities that prevent them from obtaining the skills and resources needed to become an independent adult.

ESSENTIAL FUNCTIONS:

· Adheres to St. Charles Youth & Family Services and contracted entities Policies and Procedures.

· Supports the goals of the agency mission/vision.

· Work in conjunction with team at Owen’s Place and other Community Drop-In Centers throughout the city.

· Must be able to meet with clients face-to-face, as needed.

· Prepare all documentation according to program requirements and time frames.

· Facilitate individual and group sessions with program members related to identified resources and basic needs, i.e. financial literacy, employment readiness, education options, wellness, etc.

· Provide supports to obtain & maintain employment i.e. transportation, vital statistics information, etc.

· Complete assessment and intake documents for enrollment & service planning purposes.

· Complete surveys to identify barriers and identify solutions.

· Outreach to community programs, churches, schools and centers to promote and recruit members and partners.

· Provide case management and follow up services to include referrals to community based organizations

· Provide direction, guidance, and support to a caseload of a minimum of 12 members.

· Facilitate and attend training sessions on varied topics related to the target population for the consumer or community providers/partners.

· Administer program surveys for data collection purposes and program planning.

· Assist members in obtaining skills & services geared toward independence

· Host & participate or set up community events; i.e. job or resource fairs etc.

· Attend team meetings and participate in service sessions with consumer’s team members as needed to fulfill service plan.

· Crisis intervention as needed.

· Participate in agency and community partner committees and events

· Other duties as needed and assigned.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:

· Bachelor’s degree in Social Work, Human Services, Psychology or related field preferred.

· Minimum 2 years’ experience working with target population and community based organizations that serve same population.

· Bilingual is a plus!

· Group facilitation and case management experience highly preferred.

· Experience using Microsoft Windows-based computer systems and programs as well as virtual platforms.

· Knowledge of Milwaukee County (Department of Human Services, CYFS,, Milwaukee Public Schools, Milwaukee County community based youth and young adult serving agencies)

· Ability to prepare formal reports.

· Attention to detail and good organization skills.

· Ability to display cultural competence by responding respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions and other diversity factors in a manner that recognizes, affirms and values the worth of individuals, families and communities and protects and preserves the dignity of each.

· Valid Wisconsin driver’s license and insurance required.

· Ability to use vehicle for work related duties according to the agency’s insurance liability standards.

WORKING CONDITIONS

· Typical office environment

· May be required to provide services virtually using approved platforms

· In-Home setting during client visits

· Local travel as required fulfilling essential functions of the position.

· Indoor/outdoor requirements

· Exposure to lift a maximum of 30 lbs.
