Schedule: Monday through Friday 1:00 pm-9:30 pm.
Position Overview.
Oversee day-to-day patient care for identified caseload (across SUD service lines PHP or IOP) including providing primary and specialty group and individual services, coordination of treatment, referral and collateral contacts, treatment planning, case management, and biopsychosocial assessment to ensure the patient enjoys a cohesive treatment experience where all are working together toward the same goal.

Specific Responsibilities:
- Maintains appointed continuous caseload by providing individual, family, and group counseling, crisis intervention, education, and skills training (in PHP, IOP, and GOP services).
- Completes biopsychosocial intake assessments and appropriately determines the level of care per the ASAM Criteria both in person and virtually upon request.
- Charges and collects co-payments, self-pay, or no-show fees before each point of service.
- Monitors oversight of clients’ substance use by overseeing random drug screenings and addressing results with clients.
- Collaboratively develops and updates an individualized treatment plan with the individual that includes all identified problems, goals, and discharge planning in a SMART format.
- Defines all relevant community sources affecting the care and treatment of the client (e.g. legal, family, outside providers.) and communicates with those sources to coordinate care and continued treatment.
- Updates referral sources, primary care doctor/other providers, and family members on patient progress, if/as approved by the individual in services.
- Utilizes and incorporates evidenced-based practices and standardized screening tools into treatment to engage clients in services and facilitate treatment progress.
- Maintains client medical records for compliance and quality per company and regulatory requirements.
- Meets units of billing requirements for PHP (50 Units per week), IOP (70 Units per week), GOP (30 Units per week), or a combination of the above based on census and RCA needs.
- Completes at a minimum one evening/weekend shift to accommodate night programming, with increased coverage per client needs.

Minimum Qualifications
- A Master’s degree in Social Work, Mental Health, Addictions, Psychology, or a related field.
- License to practice independently preferred (LCSW, LICSW, LMFT, LMHC).
- Licensure is required for GOP and preferred for PHP/IOP.
- At least 2-3 years of experience in a substance abuse setting serving culturally diverse populations.
- Advanced knowledge of the basic principles, philosophies, practices, and procedures involved in substance abuse treatment, including the biopsychosocial characteristics and manifestations of chemical dependency and/or mental illness.
- Ability to demonstrate competency in medical record documentation, specifically DAP group and individual notes to support utilization reviews.
- Demonstrates knowledge and ability to apply the ASAM Criteria, DSM-V, and other standard screening and treatment-related tools for placement and movement along the continuum of care.
- Must be able to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks to meet deadlines and stay organized.
- Working knowledge of State and Federal statutes regarding confidentiality laws and applicable regulations and standards.
- Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, EHR, and PowerPoint).
Work Environment.
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.
Physical Demands.
While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use their hands to finger, handle or feel objects, tools, or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus.
