Hospice El Paso is the most experienced (40+ years) and largest non-profit hospice organization in the community. We were founded by El Pasoans for the sole purpose of providing specialized care, comfort, and support to persons with a terminal illness, regardless of their ability to pay.

Our main focus is maximizing the remaining quality of life for the dying, while also providing support for their caregivers and loved ones. Specialized teams provide compassionate comfort and care and are comprised of physicians, registered nurses (RNs), certified nursing assistants, social workers, chaplains, plus admissions counselors and support staff.

The majority of these services are provided at home. Hospice care can also be provided in nursing, foster, and assisted living homes; along with rehabilitation centers and skilled nursing facilities.

The “Center for Compassionate Care” is the city's first stand-alone “hospice in-patient facility.” We provide acute hospice care in a quiet and comfortable setting. In this home-like setting, patients receive 24-hours of specialized skilled care when pain and symptoms cannot be managed in a home environment.

The Nurse Practitioner (NP) works in collaboration with the Palliative Care Team to provide palliative care services to patients and families within the scope of practice of a nurse practitioner. The NP performs procedures for which they are privileged to do and documents assessments, evaluations, consultation reports, and progress notes.

Minimum Requirements:

  • Graduate of an accredited Geriatric, Adult or Family Nurse Practitioner Master’s program.
  • Board Certified APN (as a Geriatric, Adult or Family Nurse Practitioner preferred).
  • Two years of experience as a nurse practitioner in clinical nursing care of geriatrics, adult/family practice, or oncology preferred.
  • Experience with home health, hospice and palliative care strongly preferred.
  • Computer proficiency including the ability to utilize software programs used by the organization.
  • Must be able to read, write and speak the English language. Bilingual English Spanish desirable.
  • Duties and Responsibilities:
  • Review clinical information, coordinate and oversee patient care in consultation with the palliative medical physician, interdisciplinary team, and other healthcare practitioners.
  • Consult with the patient's primary care physician, and other healthcare practitioners regarding the patient's eligibility for palliative care, ongoing care needs and medical management related to the patient's serious/terminal condition.
  • Conduct in-person and telephone visits to assess/reassess patients’ need for Palliative Care services and ongoing eligibility.
  • Manage health conditions and serious illnesses by developing an individualized plan of care, reviewing medications and/or treatments, obtaining consultations or making referrals and coordinating palliative services and/or hospice transitions.
  • Educate/counsel patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, symptom management, advanced care planning, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients.
  • Participate in the Palliative Care IDG.
  • Complete required documentation to support Palliative Care eligibility and to meet regulatory requirements for communication of findings to Hospice El Paso and Palliative Care El Paso, including documentation required for billing in accordance with Medicare and agency guidelines.
  • Communicate to Director, Medical Director and appropriate Hospice El Paso/Palliative Care El Paso staff regarding any patient needs requiring medical intervention. Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols.
  • Coordinate visits and scheduling with Director.
  • Maintain compliance with all regulatory and accrediting agencies
  • Demonstrate an appreciation and understanding of the philosophy, mission, and service standards of Palliative Care El Paso and for providing excellent customer service.
  • Liaison with palliative care teams in community hospitals and participate in any palliative care meetings at the hospitals as requested
  • Conduct face to face visits if needed for transfer to hospice service.
