NURSE-LHRS Job at KidsPeace
This position is responsible for providing nursing services to children and youth, medical consultation to FCCP staff and foster families, and following the mandated and recommended health services activities needed to support optimal health for children in foster care. Nurses must comply with Early and Periodic Screening, Diagnostic and Treatment (EPSDT) standards and state regulations while performing functions noted below.
QUALIFICATIONS:
Satisfactory completion of Mandated Reporter Training.
Licensed Practical Nurse or Registered Nurse required.
First Aid and CPR certification required.
Obtain satisfactory background checks pursuant to NY Health Care Facility requirements.
Must be at least 21 years of age.
Valid State Driver's License required.
JOB DUTIES:
Conducting assessment, including: HIV & Intake Assessments, general 1st Aid and triage activities.
Screenings for Substance abuse, developmental and mental health and physical body checks for abuse.
Training of health education to parents, staff and children, including reproductive health. Educating Caregivers, monitoring healthcare needs and assisting with home visits.
Medication administration and management.
Assisting with Community provider visits, follow up with any and all medical appointments or services related to health. Facilitate communication between the voluntary agency and the LDSS so that the district has the right amount of information to understand the child’s health concerns and properly oversee safety and well-being.
Establishing Treatment Goals based on assessments, screening and historical information while also working with an inter-disciplinary team.
Monitor the entry of data into the CONNECTIONS Health Services Module. Keeping documentation timely, comprehensive, and accurate. Ensuring the health file is legible, organized, easily useable and available for continuity of care, monitoring, and oversight.
Practicing infection Control, ensuring adequate nursing staff coverage and maintaining medical supplies and equipment.
Must be available on a 24/7 basis to respond to any urgent needs these children and youth may have and should also be available to address any concerns or issues raised by State, City, and County authorities.
All other duties as assigned by management.
