1. HHA Certificate (40 Hours) or Current CNA License
2. Current CPR Card (American Heart Association & Red Cross)
3. Original 4 Hours HIV/OSHA Certificates
4. Work Authorization
5. Professional Certificate of Insurance with Liability Coverage if you do not have you have the option to payroll deduct.
6. Social Security Card
8. Current Driver’s License
9. Current Car Insurance if applicable
10. Physical Exam within 5 months old or less. Must state you free of communicable disease.
11. COVID Vaccination Card (collected if available, but not required)
