Manager, Outreach & Testing Job at Heartland Alliance

Summary: This position provides coordination of shelter-based health care to special populations at a variety of outreach locations throughout the north, south and west sides of Chicago for Heartland Alliance Health (HAH). This person will work with partner agencies (schools, community organizations, etc.) to develop collaborative service delivery models unique to participant needs, in order to improve health outcomes of population. This role is instrumental in fostering strong existing partnerships with HAH and in developing new partnerships with community organizations, such as shelters for people experiencing homelessness and other homeless service provider agencies is a key responsibility for this role. This role will work in partnership with the Heartland Alliance DECM team. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.
Essential Duties and Responsibilities:

  • Plans, coordinates, and ensures successful implementation of the agreed upon services that our outreach health team will provide at the partner’s site.
  • Responsible for creating and maintaining dynamic schedules for health care providers and support staff to provide testing (COVID-19, Diabetes, etc.) and on-going shelter based care, including telehealth at numerous outreach locations.
  • Educates partner agencies about all services available to participants at HAH and provides linkages to care when appropriate.
  • Is the site leader for the outreach health team while on the partnering agencies facilities.
  • Partners with behavioral health outreach liaison to engage participants in behavioral health care services as requested/needed. Assists with research, planning, and development for outreach site expansion and program sustainability.
  • Assists in representing HAH at homeless service meetings and functions (committees, planning groups) as assigned and/or approved by the Director.
  • Assists in convening non-affiliated organizations in Chicago that provide supportive services to the community including coordinating meetings, taking minutes, and arranging presenters when assigned and/or approved by the Director.
  • Establishes and maintains professional relationships with groups and organizations that provide social services to homeless populations under the collaboration of the Director.
  • Partner with the HA communications and marketing team to provide content updates on HAH’s impact and service offerings for inclusion in various comms/mktg channels – social, website, e-new, etc.
  • Prepares flyers and other marketing materials to promote services.
  • Facilitates communication initiatives upon direction of Heartland Alliance.
  • Work to identify viable customer segments/sectors prospects – schools, community organizations, etc.
  • Remains current in the field of service provided by the program through self-initiated avenues.
  • Ensures the accuracy, content and timeliness of program reports, projects, and other relevant work projects.
  • Participates in team meetings as assigned and/or approved by the Director.
  • Adheres to ethical and professional standards as outlined in the HAH operating procedures as well as under the direction of the Medical Director and/or SBC Lead Provider to ensure staff and participant safety while providing HAH services at outreach sites.
  • Other duties may be assigned and are in alignment with this role’s work in serving HAH’s mission.

  • Education and/or Experience:
  • Bachelor degree required in social work, public health, social services, humanitarian, and/or community health and three years related experience in one of the above concentrations or an Associate’s degree with five years related experience.
  • Exceptional organizational skills, strong interpersonal communication, skills, highly relational and ability to work in a dynamic, fast paced environment necessary.
  • Must be comfortable with working in shelters and other community based settings as well as traveling to outreach sites both in Chicago and the suburbs.
  • Must have a valid driver’s license with at least three years active driving record and have their own transportation.
  • Experience in mental health, social work, community health or related field preferred.
  • Marketing and Customer Acquisition experience a plus.
  • Language Skills: Fluency in a second language preferred.
  • Work schedule requirements: Different shifts as determined by needs of the program and can fluctuate as needed. Workdays will generally begin no earlier than 7:00 a.m. and end no later than 9:00 p.m. Some Saturdays may be required.
