Waveny LifeCare Network is currently looking to hire Licensed Practical Nurse (LPN) for our skilled nursing and assisted living divisions. The LPN position plans, organizes, and directs services and is experienced in nursing, with emphasis on community health education/experience. The position builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.Responsibilities will include but are not limited to:

• Provides quality patient care.

• Administers medications as ordered by the physician in accordance with Waveny Care Network policy assuring patients receive the correct medications and doses while understanding all aspects of medications.

• Has knowledge of patients’ conditions at all times reporting changes to the appropriate discipline recognizing all physical and psychological conditions.

• Implements patient care according to the medical/nursing care plans assuring sound, accurate and comprehensive care is delivered to each patient.

• Provides health care instructions to the client as appropriate per assessment and plan of care.

• Attends all staff meetings or reviews agenda to keep abreast of changes/updates.

• Participates in staff meetings to share skill set, ideas, and suggestions.
• Documents according with prescribed agency tools in timely fashion.
• Prepares clinical notes and updates the primary physician when necessary and according to policy.
• Qualification for this role:

• Graduate of an accredited school of nursing. Bachelor’s degree preferred. One (1) to two (2) years of recent acute care experience is an institutional setting is preferred or recent home care experience.
• Current licensure in state, CPR if applicable and valid driver’s license. Able to provide supervision to CNA;s and home health aides.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.
• Computer knowledge helpful but not required.
• Prolonged or considerable walking, standing. Able to lift position or transfer clients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills. Driving in all weather conditions.
