Intake Coodinator (Per Diem) Job at Zenith Behavioral Health

Position Summary:
Under the direction of the Intake Director and working closely with the I multidisciplinary team the Intake Coordinator ensures that inquiries received from individuals seeking information about behavioral health services are provided with all the information required which includes eligibility for services. Communicates with referral sources or potential patients, schedules and handles initial intake appointments, conducts clinically appropriate assessments to determine appropriate level of care. Presents consistent, effective patient centered communication welcoming new patients during the admissions process.
Essential Duties:
- Coordinates admission to treatment for patients.
- Arranges for admission by coordinating financial, clinical and other pertinent information including insurance verification and treatment authorizations.
- Documents all received telephone inquiries from interested parties requesting information and assistance regarding inpatient services. Working closely with Intake Director, prescreens for behavioral health emergency conditions and makes appropriate referrals as needed.
- Provides concise, relevant clinical information about patient to multidisciplinary team responsible for patient care, documents clear and concise information into data system-EHR.
- Utilizes crisis assessment and crisis intervention criteria (as needed) to conduct clinical assessment and documents information for review with appropriate clinical team to determine level of care required for patient.
- Maintains knowledge and establishes contacts with community resources.
- Communicates and interacts effectively as a member of a team with management and staff at all levels and works collaboratively with others.
- Keeps management team aware of pending discharges, admissions and expected census.
- Key member of site leadership team, attends scheduled meetings, provides required reports, provides important patient admissions information, manages multiple tasks and successfully meets deadlines.
- Provides and supports staff training on relevant intake and admissions criteria to improve understanding by staff about patient admission and patient care services requirements.
- Maintains a state of continuous regulatory readiness.
- Provides information to Intake Director as appropriate, to improve the quality of services and to resolve identified problems.
- Communicates identified process improvements and new system initiatives relevant to intake/admissions procedures.
Qualifications:
- Bachelor's degree in Social Worker counseling required, or RN with Arizona RN license.
- A minimum of 2 years of experience in behavioral health inpatient healthcare intake/admissions services.
- Must be 21 years of age +, must have a valid driver's license, a Level I AZ DPS Fingerprint Clearance Card, and able to pass required clearances, background checks.
- Must be flexible to adjust to the needs of a fast-paced environment, demonstrate effective problem-solving skills, and display proficient oral/written communication skills
- Must be able and willing to work a flexible schedule, including weekends, holidays, shift changes.: Candidate must be a dynamic, self-motivated person with excellent interpersonal and leadership skills.
