Job Summary

Under general direction, is responsible for performing complex professional level projects that require a high degree of specialized knowledge. Conducts research, provides data, makes recommendations, and implements solutions to achieve project goals and objectives. Working conditions are primarily inside an office environment. May exercise supervision over assigned staff.

The San Antonio Healthy Start (SAHS) SMC will provide high-level administrative and program support to the City of San Antonio (CoSA) Metropolitan Health District’s (Metro Health) Healthy Start Program Director and/or Manager. The SMC will directly oversee SAHS case management and fatherhood program areas and assist with coordinating the Healthy Family Network (HFN) coalition. Along with high-level administrative duties, including but not limited to oversight of the multidisciplinary SAHS team, maintaining client records, completing required reporting, and grant management, the role may require the SMC to serve as a health educator and serve alongside case managers to promote and educate clients in-office, in-home and in an individual or group setting. This role requires an ability to be flexible, quickly adapt to situations, and a willingness to step into various positions and lead or complete diverse tasks to ensure the effective functioning and performance of SAHS and Metro Health.

This position is a "grant funded" position, therefore unclassified and has no disciplinary appeal rights to the Municipal Civil Service Commission. This position is likewise subject to at-will employment status and can be terminated at any time for any reason.

The San Antonio Metro Health District (SAMHD) provides public health services to clients, which may include risk for exposure to and possible transmission of vaccine-preventable diseases. Employees in high-risk positions will be required to obtain relevant immunizations, per SAMHD Policy DM13.2 (Download PDF reader). 7:45 a.m. - 4:30 p.m., Monday - Friday; some evenings, weekends, and overnight travel required


Essential Job Functions

  • Undertakes special project efforts assigned by the Management Team.
  • Performs efficiency improvement analysis on certain City functions and services.
  • Performs specialized analysis, including cost of service and performance evaluation, and makes recommendations for solutions to problems.
  • Assists departments in compiling, organizing and presenting proposals to Management Team and City Council.
  • Coordinates projects and activities between various divisions, City departments, public agencies and/or private sector representatives to include, conducting monitoring and evaluation of case management performance and projects/activities and design and implement quality improvement and assurance projects and plans in conjunction with the HS Program Manager (PM) and External Evaluator.
  • Seeks and evaluates information from a variety of sources; provides specialized reports as requested by management and ensures ongoing rapport, communication and collaboration with current program partners, funders, community members and other stakeholders and establish new collaborative relationships by serving as an MCH, SAHS, Metro Health and CoSA representative.
  • Oversees coordination between divisions/departments throughout duration of projects and serve as supervisor to the Healthy Start Social Service Manager (SSM) to help manage the effective coordination of case management staffing complement and direct services; conducts staff meetings in collaboration with the PM to ensure continuous and intermittent accomplishments are made and recognized for all program services and activities.
  • Organize and facilitate case management staff meetings to ensure the proper functioning of case management services, including but not limited to community outreach, recruitment and enrollment; health education; home visitations; counseling and other mental health/social services referrals, and monthly staff auditing report process; prepares and delivers presentations; organize, implement, and ensure compliance for staff training as needed for SAHS program, including mandatory Metro Health, CoSA, or grantor items; and creates and delivers presentations and grant reports as needed for various uses, including CoSA, departmental and MCH leadership, internal and external partners, and grantors .
  • Collaborate with SSM to review and follow up on case managers’ (CM) audit reports/performance and liaise with Office Manager, SSM and PM regarding incentives and supply inventories to support case management activities; coordinates and prepares reports and correspondence in response to requests for information from City Manager's Office and Mayor & Council Offices; may also prepare City Council agenda material such as ordinances, council memos, etc.
  • Performs related duties and fulfills responsibilities as required.

Job Requirements

  • Bachelor's Degree from an accredited college or university.
  • Four (4) years of increasingly responsible experience in municipal government, business, or a related field.
  • Any applicant selected for this position must have a background check completed with results satisfactory to the City of San Antonio and successfully complete the City of San Antonio's Cash-Handling Certification Training.
Preferred Qualifications
  • A Master's Degree from an accredited college or university with major coursework in Public Health, Community Psychology, Sociology, Social Work, Epidemiology, or related field.
  • Three years of experience in a Public Health, Social Service, Community Organizing, or Health Advocacy, to include administrative or supervisory responsibility.
  • A subject matter expert in women's, child, and family health.
  • A leader in engaging diverse community partners and building and leading community coalitions or consortia.
  • History of successfully coordinating staff and activities to ensure compliance and quality of community health performance measures, including demonstrated performance with specific target populations.
  • Skilled in program and policy development, contracts management, implementation and reporting.
  • An expert in utilizing multiple data sources to develop strategy and showcase program success.
  • Fluent in English and Spanish.
  • Professional experience with case management, including supervision and/or corrective action intervention over case management and care coordination activities.
  • Valid Class "C" Texas Driver's License OR valid driver's license from another state with the ability to obtain a valid Class "C" Texas Driver's License within 30 days of becoming a Texas resident. Use of your own personal vehicle may at time be required and proof of Liability Insurance will be required.
  • Knowledge of women's maternal, child, and family health.
  • Skill in utilizing case management software and performing quality assurance on case management documentation.
  • Ability to develop and implement policies and procedures.
