Health Home Care Manager 2-

• Assess and identify client need in conjunction with client, referring agent, family members, etc.
• Identify and determine resources that are available to clients within Warren and Washington Counties.
• Coordinate and link clients to resources and service providers in the community.
• Actively intervene and advocate on behalf of clients as needed.
• Provide or coordinate transportation for clients to necessary appointments.
• Provide follow-up services to client requiring additional support and crisis intervention.
• Provide supportive counseling to clients as appropriate.
• Maintain monthly contact with assigned members.
• Participate in weekly staff meeting for the purpose of program planning and coordination of client’s services.
• Maintain program documentation as required.
• Adhere to appropriate Medicaid billing requirements.

Qualifications:

Education:

• Associates Degree with 3 years of experience OR
• High School degree with 6 years of experience

Experience:

• Providing direct services to persons with serious mental illness, developmental disabilities, alcohol or substance abuse; OR
• Linking persons who have serious mental illness, developmental disabilities, alcohol or substance abuse to a broad range of services essential to successful living in a community setting (e.g. medical, psychiatric, social, educational, legal, housing, and financial services
