Entry Level Human Services Coordinator Job at The Coordinating Center

If you are looking to continue your human service experience in the service coordination field, an entry level position with us might be match for you! You will be working alongside our Supports Planners to help coordinate services for our clients, with the goal of transitioning to become a Supports Planner after a few months of training. (Please see job description for Supports Planner). As you continue through training and orientation, you will become more familiar with the different community services associated with various Medicaid programs, while navigating our electronic tracking system LTSS.


Essential Responsibilities

  • Triages calls and other inquiries from clients, families, providers, and outside agencies.
  • Completes monthly client contacts as assigned, addressing any issues or concerns.
  • Navigates the electronic tracking system LTSS to complete client-related activities including all documentation and billing requirements.
  • Locates available community resources for clients and follows-up for additional assistance.
  • Processes any administrative coordination tasks including application forms, reports/spreadsheets, DME/EM referral process, chart reviews, etc. and updates electronic client files.
  • Transmits information and documents to coordinators, providers and clients using email and e-faxing.
  • Prepares legally required and special reports while adhering to all federal, state and local regulations.
  • Demonstrates the ability to respond to changing situations in a flexible manner to meet current needs, reprioritizing work as necessary.
  • Takes initiative to provide support to other team members and assist in activities that will meet the needs of The Center.

Qualifications

  • Bachelor’s degree in health, human services or related field is preferred.
  • At least one year of human services experience working with clients.
  • Ability to communicate effectively with others to establish professional relationships and ensure customer satisfaction.
  • Ability to exercise good judgment to take initiative and act proactively.
  • Excellent organizational skills.
  • Ability to manage multiple priorities and the flexibility to adapt to changes that occur with an evolving position.
  • Proficient in Microsoft Office Suite (i.e. Access, Excel, Word, PowerPoint, Outlook).
