Position Title: Case Worker

Overview: The Case Worker serves as a resident advocate and liaison between the facility and insurance companies. This individual will be responsible for thorough communication with residents, family members, case managers, and departmental staff. This position works in conjunction with the social worker and only works with HMO plans.

Essential Job Functions:

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

• Visits residents within 72 hours of admission to assess the transition plan.
• Identifies insurance for each resident to determine coverage of services and to meet case manager.
• Contacts case managers from different insurance companies within the first week of admission.
• Provides all requested documentation, (e.g. rehab and wound care notes, follow-ups, and medical concerns), to insurance case manager.
• Shares updates with insurance case managers on a weekly basis.
• Requests any equipment or home health services for residents (e.g. wheelchair, commode, physical, occupational, or speech therapy, wound care).
• Informs resident of length of stay and provides discharge information 3 days prior of discharge date and non-coverage letter provided or requested by insurance company.
• Informs social worker if resident decides to appeal non-coverage.
• Documents all information pertaining to the resident.
• Attends all meetings regarding the status of each resident.

You Have:

• Bachelor’s degree in case management, social work, or related healthcare field preferred.
• Minimum 2-3 years of case worker experience, preferably in a senior care or CCRC facility.
• CCM certification preferred.
• LPN or RN license preferred
