Compensation (Pay): $104,618.13 to $151,696.29/annually, based on experience.

Job Summary: The Director of Population Health will serve as a leader, collaborating with clinical and care coordination teams to create and maintain an environment of high-quality care and care coordination services. The Director of Population Health understands and operationalizes quality measures and facilitates performance improvement teams to achieve compliance with each measure set. This position functions as an objective role that reviews and evaluates quality and satisfaction issues/concerns within the health centers. Oversight of Care Coordination department to ensure complex coordination services are in alignment with RAE/Medicaid contracts.

Essential Duties and Responsibilities:
  • Requires excellent communication skills both oral and written. Ability to generate, interpret and analyze data from multiple sources
  • Collects and analyzes data and produces quality reports that are useful and meaningful, with comparisons to national benchmarks and assures systems are in place to maximize quality incentive program revenues and scores.
  • Identifies quality and care coordination initiatives that are evidence based and develops programs to implement best practice initiatives
  • Develops strategic plans and policies for improved quality throughout all Peak Vista Health Centers with PDSA/Root Cause Analysis platforms
  • Indirect Supervision of Quality RN's, Health Informatics and Care Coordination staff to include Care Coordination RN's as well as Lead Care Coordinators and Care Navigators
  • Receives and directs quality issues to appropriate resources for investigation and resolution to include documents and other information relative to quality activities, including but not limited to, patient records
  • Oversee quality care coordination services provided to all patients
  • Monthly Quality Reviews for monitoring of trends and patterns that can affect Value Based Care Contracts
  • Onboarding of new clinical staff to better understand quality metrics and healthcare economics
  • Yearly review of RAE/Medicaid contracts for edits related to quality and care coordination payouts and requirements
  • Coordinates partnership with internal and external stakeholders for continuity of care
  • Ensure that care coordination referrals are worked in a timely manner as outlined in our contracts, ensuring timely assignment and outreach
  • Participate in monthly RAE, TRE, CHPA, PIAC, etc meetings and be primary contact for quality and care coordination questions/concerns
  • Responsible for Medicare Shared Savings initiatives and reporting
  • Responsible for RAE/Medicaid KPI initiatives and reporting
  • Consult and collaborate with clinical teams, providers, administration and community-based healthcare workers and agencies regarding care management issues identified
  • Leadership support for complex patients transitioning from Hospital to Primary Care and address barriers to prevent readmissions and patient complications
  • Leadership support for complex DHS cases to ensure care coordination services are addressed correctly as required in contract
  • Demonstrates knowledge of and adherence to the Compliance Plan and conflict of interest requirements.
  • Enhances the effectiveness; efficiency and productivity of the department by contributing to, and participating in, departmental performance improves goals. Be a fiscal steward of resources
Supervision Exercised: Demonstrates strong leadership skills, knowledge, and abilities. Able to have difficult conversations with providers and staff to ensure quality patient care isn't compromised. Carries out supervisory responsibilities for Care Coordination Manager, Quality RN's, Care Coordination RN's and Health Informatics Coordinator. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Ensures that the appropriate personnel meet all job required competencies.

Education/Experience:
Education: Bachelor of Science in Nursing required. Masters Business Administration (MBA) preferred.

Work Experience: Eight to Ten years of nursing experience in a hospital or ambulatory clinic setting required. Five years' experience managing, and leading quality improvement programs/initiatives/care coordination and personnel required. 5 years in management required.

Computer Skills: Intermediate - ability to access the intra/internet to manage timecard, review policies and procedures, and read company communications; use e-mail to communicate with co-workers, leadership, and other departments; use and create a variety of templates, complex tables, merges; manage table data, sort and filter merges, and also perform basic work with existing Macros; customize toolbars, import and insert graphs, embed Excel data, and elaborate reports; work with multiple worksheets, filter data, use integrate functions, and manipulate databases; customize templates and the PowerPoint environment, and to make a presentation interactive by using hyperlinks and action buttons

Certificates and Licenses: Current RN license. Informatics Nursing Certification preferred. Current CPR (BLS). Basic knowledge of CPT, ICD-10 and HCPC codes preferred as well as individual insurance requirements. Previous grant experience preferred
