Under direction of the Program Manager of the Psychiatric Triage and within federal, state, and local guidelines provides moderately complex case management work. Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. This position is grant funded.

Coordinates monitors and provides crisis assessments, treatment planning process and follow up for individuals in the psychiatric triage facility, serves as an advocate and liaison on behalf of the individual served. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.


Essential Duties

Greets individuals in psychiatric triage facility and gain an understanding of what brought them to the facility.

Collaborate with Psychiatric Triage team member to determine least restrictive treatment through screening form and other assessment methods

Facilitate with individual and with community agency to carry out least restrictive environment.

Uses Child/Adolescent Needs/Strengths (CANS) or Adult Needs Strengths Assessment (ANSA) to determine if individual qualifies for crisis package services. (LOC0 and LOC9).

Works collaboratively with MCOT team

Develop and implement an individual crisis plan for each individual that incorporates diagnosis, individual/family preferences, intervention, outcomes, follow-up plans and referrals for 100% of individuals assessed.

Provides thorough documentation

Provides telephone follow ups to individuals as needed and as directed by Denton County MHMR daytime MCOT teams. Instructs, counsels, and assists clients in meeting their health care needs. Provides symptom management and education with the focus of resolving crisis while assisting the individual to be an active participant in their treatment.

Develop and implement an individual crisis plan for each individual that incorporates diagnosis, individual/family preferences, intervention, outcomes, follow-up plans and referrals for 100% of individuals assessed.

Ensures follow-up on 100% of clients screened within 24 hours.

Attends regularly scheduled meetings.

Provides emergency intervention when necessary according to Department of Human Services (DHS) and Center rules and procedures.

Perform additional duties as deemed necessary by department supervisor within designated timeframes.

Perform job duties in a safe manner and report any unsafe practices or conditions and demonstrates awareness of potentially dangerous situations.

Ensures completion of tracker on each shift.

Ensures caring letters are sent out appropriately.

Keep daily service logs on all direct care activities.



Education and or Experience
Graduation from an accredited four-year college or university with major course work in a human services field as required to qualify as a Qualified Mental Health Professional (QMHP) and one (1) year prior experience in a mental health field required. Master’s Degree in human services filed and previous Crisis experience preferred. English/Spanish bilingual preferred.
