Job Purpose: A Community Service Provider delivers a variety of services through various funders to clients with co-occurring disorders in their homes and the community at large to develop skills and/or maintain the capacity to achieve or maintain integrated independent living. The provider primarily uses teaching, coaching, mentoring and supervising techniques to assist individuals to achieve personalized goals, preventing the need for more costly supervised care of structured treatment. Services include, but are not limited to, medication management, individual daily living skills and specialized parenting assistance.

Reporting Relationships: This position reports to the Residential Case Manager and has no direct reports.

Essential Job Functions:

  • Provide a full range of independent living skills and supportive services; promotes the client’s choice and preparation in his or her overall program and assists the case manager with assessment and plan development as defined by the contract.
  • Perform and properly document service delivery on a timely basis.
  • Maintain service level and appropriate contact with client and external stakeholders (i.e. funder, external case manager) to ensure clients’ needs are met and service/treatment plans are followed.
  • Teach and assist clients with supportive and independent daily living skills, including but not limited to, approved nurse delegated duties, personal care, housekeeping and home maintenance, mobility and transportation, interpersonal relationships, safety practices, financial management, decision making, utilization of community resources, assessing appropriate professional medical services, parenting skills, budgeting and business transactions, obtaining financial and medical assistance such as SSA, SSI, and Title XIX, self-care and leisure skills.
  • Teach, remind, administer and monitor client in medication adherence as directed by their physician.
  • Report to program management, program nursing support, program case manager and Client’s physician as appropriate, changes in client behavior, functioning, or condition.
  • Promote the image of the agency and the benefits of the program and the agency by participating in professional networks, and affiliated organizations; present at internal and external conferences and trainings.
  • Practice and promote safety in the work place and community. Demonstrate sound judgement when dealing with safety issues particularly in community settings and report all incidents/accidents according to agency policies and procedures.

Required Technical Skills, Experience, Education, and Credentials:

  • Must have a minimum of a High School Diploma or equivalent. Associates degree in Social Work or other related field preferred.
  • Must have at least 6 months experience working with individuals with mental health issues, physical health issues, and/or dementia.
  • Preferred candidate would have degree/certificate (CNA, CBRF) and experience with mental health, physical health, dementia, and/or outcomes based programs.
  • Current Wisconsin Driver License, without limitations that are incompatible with duties & responsibilities, and automobile insurance with minimum limits of $50,000/$100,000 bodily injury and $10,000 property damage.
  • Access to personal vehicle.
  • Knowledge of rehabilitative needs of people with various kinds of disabilities.
  • Demonstrated proficiency using a laptop and phone applications. Ability to navigate through a variety of Microsoft programs and ability to learn multiple different computer programs.
  • Must be able to effectively communicate with MCFI Home Care staff, external case managers/care coordinators, clients and stakeholders in professional manner.

Work Environment/Physical Requirements: The work environment and physical demands described here are representative of those that an employee may experience or must do to successfully perform the essential functions of the job:

  • Must be able to travel via personal vehicle into various communities throughout Milwaukee and the surrounding areas in any weather conditions that don’t present immediate danger.
  • Must be comfortable going into potentially unfavorable home conditions with possible exposure to pets and pests.
  • Must be able to push, pull, lift, and move objects weighing up to 50 lbs. and safely transport a client in a wheelchair on a level flor of wheelchair ramp in various weather conditions.
  • Must be able to continuously cope effectively with stress created by participants with cognitive delays, complex health and mental health care problems, multiple tasks, noises, interruptions, and work cooperativity as part of a team while maintaining a positive demeanor.
  • This position requires flexibility with hours in the evening and on weekends as needed.
