We're looking for a motivated individual to lead nutrition education in a human services environment.

Overview: The Community Nutrition Educator will develop and deliver group and individual nutrition health education that incorporate goals and objectives on HIV specific health and nutrition topics.

Status: Full-Time (37.50 hours), Hourly,$50-$55,000/annual, Benefit Eligible

Minimum Qualifications:

1. BA or BS in Nutrition or related field such as health education.

2. 1-2 years experience developing and facilitating nutrition health education.

3. 1-2 years working with individuals with chronic health conditions. HIV/AIDS experience a plus.

4. Possess an understanding of community level work and the importance of collaborating with other organizations.

5. Excellent communication, writing, and computer skills.

6. Cultural and linguistic competence for the target population. Spanish fluency a plus.

7. Clean driver’s license, ability, and means to travel on a flexible schedule as needed.

Responsibilities:

1. Develops nutrition health education plans that incorporate goals and objectives, frequency and duration of the sessions, HIV specific health and nutrition topics.

2. Adheres to AIDS Institute requirements for the provision of Nutrition Health Education (NHE).

3. Reviews RD assessment and dietary recommendations to determine the food and meal service client receives; and type of health education appropriate to meet needs.

4. In conjunction with the Program Manager, monitors the appropriateness of the food and meal service provided to clients.

5. Establishes connections and communicates frequently with Medical Nutrition Programs/Practitioners, and RDs and makes referrals as necessary and appropriate.

6. Completes NHE and food and meal screening forms, including the reasons for client participation in the nutrition health education program.

7. Plans and facilitates a case conference on each client who has been in the NHE program for six months, more frequently when necessary.

8. Maintains electronic client records that document attendance, participation, progress, accomplishments, additional needs, referrals and goals.

9. Ensures client adherence to NHE food and meal policies and procedures, follows protocol for food gift card/voucher distribution.

10. Acts as Team Leader with Nutrition Program Assistant to conduct community coordination activities.

11. Communicates and collaborates with AIDS Institute staff and responds timely and appropriately to all requests.

12. Attends all AIDS Institute required meetings and trainings.

13. Fully engage in self-care to promote physical, emotional, and spiritual well-being.

14. Attend full staff meetings and meet required 24 hours of annual training.

15. Other duties and special projects as assigned.
