Job Summary
- CYPHER (Community of Young People Healing, Experiencing, and Rebuilding) groups, a culturally relevant group intervention based closely on the evidence informed S.E.L.F. (Safety, Emotions, Loss, Future) trauma recovery groups, which have proven effective in mitigating the effects of trauma. These concepts are interwoven with trauma education and coping skills related to both a group member’s current injury, as well as his or her lifetime adversity.
- Individual Behavioral Health Treatment: The CHWP will also provide linkage to community resources and information about and referrals to more structured trauma therapy through the HHP program. The CHWP position requires the ability to engage with individuals in a trauma-informed manner, and to model effective ways of engaging with resources that support healing. This job requires a significant amount of community-based work, and willingness to travel to each partner site on a regular basis.
Essential Functions
- Assist COT’s Program Manager with guidance and mentorship of COT’s CHWPs
- Provide direct supervision of at least 1 Healing Centered Learning (HCL) Intern.
- Act as key contact for staff at partner sites and involved in developing specific interventions at each site.
- Outreach, Engagement and Support: The CHWP will work to outreach, engage, and maintain connection with young people impacted by violence. • Provide unique support, and modeling of positive behaviors to a range of 24 to 36 participants on a weekly basis. Support of family members may also be a component of this position.
- Navigation of Services: The CHWP will also serve as a role model serving the recovery and wellness of participants receiving services, including setting personal goals and connection to internal and external resources.
- Group Facilitation/ Role Modeling: CHWP may assist in facilitation of informal meetings at program sites, including relationship building, psychoeducational resources, and professional modeling.
- Teamwork: Collaborate with COT and HHP staff to develop the most appropriate interventions, including warm handoffs and referrals to HHP.
- Documentation: All contacts with COT participants must be maintained consistently and accurately in a web-based database. Documentation may also include tracking client attendance in group or other activities and keeping track of any experiences the CHWP may have related to COT.
- Other duties as assigned by supervisor.
Required Qualifications
- A High School Diploma or GED is required.
- Minimum of 5 years’ experience providing peer services to violently injured people is required.
- Certified Peer Specialist certification required.
- Proficiency with MS Office and prior experience using a smart phone, email and the internet are required.

- Applicants must have personal experience in overcoming violence related injury, either to
- Ability to work independently and as part of a team.
- Sensitivity to and compassion for the diverse experiences of individuals impacted by violence.
- Willingness to encourage and inspire others to make positive changes and help them find
- Experience in facilitating psycho-educational groups preferred.
- Must be able to present self and program with high integrity, be punctual and reliable.
- Ability to take constructive criticism and work well with supervision.
- Direct experience as a participant of a violence intervention program is preferred, as is prior experience helping other individuals make lifestyle changes and/or experience
- Participation in the Community Health Worker Peer (CHWP) Training Academy is preferred.
