Position Description
The Collaborating Physician, Specialty Programs works with an APN (or more than one) who is performing physical medicine and rehab assessments on patients in the skilled nursing home setting. APNs working with the Collaborating Physician will not have prescriptive authority, nor will they be treating patients from a primary care perspective.
Essential Duties and responsibilities
- Be available via telecommunication or in-person for consultation with APN to address patient assessment questions
- Regularly performs annual chart reviews
- Consults with APN ensure adherence to policies, procedures, state laws and quality
- Other duties as assigned
- Accountable for outstanding customer service to all external and internal customers
- Develops and maintains effective relationships through effective and timely communication
REQUIRED Knowledge, Skills and Experience
- Active/unrestricted Medical Physician License in the state posted
- Must be able to relate professionally and positively to physicians, providers and staff with excellent communication, interpersonal and management skills
Preferred Knowledge, Skills and Experience
- Experience in geriatrics or complex patients preferred
