Description:The clinical pharmacist will be responsible for executing clinical service programs, including monitoring reimbursement rates, enrolling new patients, and RPM device management. Responsibilities may expand or vary, depending on the patient needs of each practice.

Collaborative Practice responsibilities:

  • Establish and maintain a professional relationship with all team members within the medical practice.
  • Spearhead patient enrollment, device management, and data interpretation for RPM services requested by the medical facility.
  • Communicate critical alerts to providers in a timely manner and provide supplemental data.
  • Provide recommendations or implement therapy changes, when necessary as allowed by the CPA with the provider.

Educational Responsibilities:

  • Participate in topic discussions as scheduled in supported Community Pharmacy Residency Program.
  • Precept PGY-1 Community Pharmacy Residents and pharmacy students.
  • Participate with supported Fellowship Programs.

Chronic Care Management Responsibilities:

  • Performs care management for chronically ill patients with chronic diseases such as chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, and/or congestive heart failure (there are several others).   • Creates and update patient-specific care plans
  • Work with the patients, family/caregiver(s), clinical providers, and community resources in a team-based approach to increase the patients’ ability for self-management and shared decision-making.
  • Complies with documentation requirements of CCM program by carrying out the care plan with the patient, family/caregiver(s), and providers then recording in appropriate database(s).
  • Create an ongoing process for patient and family/caregiver(s) to determine and request the level of care coordination they desire
  • Facilitate patient access to appropriate DME providers
  • Work closely with providers to manage the day-to-day calls involving symptom control, medication management, and to provide patient and family/caregiver(s) education
  • Manage care transitions between and among health care providers and settings, including referrals to other clinicians, or follow-up after an emergency department visit or after discharges from hospitals, skilled nursing facilities, or other health care facilities

Remote Patient Monitoring Responsibilities:

  • Perform care management for chronically ill patients with chronic diseases whose vitals are being monitored remotely. These include blood pressure, pulse, blood sugar, daily weights, and/or pulse oximetry.
  • Review and manage critical alerts of patients’ vitals
  • Each patient is looked at individually. Pharmacists are trained to look at several factors and not just the parameters that are being measured. Some other parameters include, but are not limited to drug interactions, cost, timing of medications, transportation, and mental health.
  • Building rapport with the patients to help ensure compliance.
  • Initiating and adjusting medications according to patients’ vitals.
  • Assess proper device technique.
  • Management and supervision of ancillary staff such as Medical Assistants and/or Pharmacy Technicians to help them carry out CCM and RPM services

Skills:

  • Must be highly motivated, result-oriented with strong skills in presenting, communicating, organizing, multi-tasking, and time management skills
  • Strong organizational and interpersonal skills
  • Excellent communication demonstrated by positive feedback from patients/providers
  • Ability to identify problems and recommend solutions
  • Ability to read, write, and communicate effectively both verbally and in writing
  • Basic computer skills including previous experience with various databases and electronic health records (EHRs)

Education and Certification/License:

  • Graduate of ACPE-Accredited School of Pharmacy or International Equivalent.
  • Current, valid license to practice pharmacy in Georgia.
  • Completion of APhA/ASHP Accredited PGY-1 or PGY-2 Residency Program or Fellowship and/or have 5+ years of Ambulatory Care experience
  • Current CPR Certification.
  • Certification for Immunization.
