Pay Range: $58,500.00 to $66,500.00 and up depending on licensure
Sign On Bonus: $4,000.00
Riverbend Community Mental Health, Inc., one of the largest providers of community-based mental health services in New Hampshire, is seeking a full-time Case Manager Team Leader to join our talented multi-disciplinary Community Support team working with adults living with severe and persistent mental illness.
The Case Manager Team Leader works with the Community Support Program Director (Concord) to maintain high quality, well-integrated case management services for adults with severe and persistent mental illness and co-occurring intellectual/developmental disabilities. The team manages a clientele with dual diagnosis (MI/ ID) and/or young adults transitioning into adult services in the Riverbend Community Support Program. The Manager in this role oversees the provision of clinical care, provides supervision to staff, monitors risk management, and participates in short and long-range planning of services to meet the needs of adults in the program. The Manager may also provide some direct services to a small caseload of clients.
Essential Duties and Responsibilities:
- Participate in the recruitment and training of newly hired staff
- Provide clinical and administrative supervision to staff
- Work collaboratively with clinical treatment teams
- Track and manage program/team referrals and staff caseloads
- Monitor resources effectively to meet or exceed budgetary expectations
- Actively engage with program partnerships with community agencies
- Provide direct services with a minimal caseload, based on program need
- Provide individual/group therapy and conduct intake assessments
Education and/or Experience Requirements:
- License eligible Master’s Degree in Psychology, Social Work, Education, Rehabilitation, or Human Services required.
- Three years’ of direct work experience with adults with severe and persistent mental illness required. Experience working with adults with intellectual/developmental disabilities preferred.
- One year of supervisory experience is preferred.
- Excellent analytical, organizational, interpersonal, oral and written communication skills with attention to detail is required.
- Valid driver’s license, personal vehicle and auto insurance, or access to a reasonable alternative is required.
