Position: Clinical Intake/Case Manager | Footprints to Recovery, Elgin IL | LSW or LPC w/CADC Preferred |

The Clinical Intake Counselor/Case Manager assists with the admissions process while also supporting the team through the delivery of clinical services to ensure comprehensive and effective daily programming for our ever-growing inpatient and outpatient programs. Footprints to Recovery offers medically managed detox, residential treatment, partial hospitalization, intensive outpatient, and outpatient treatment. Specific responsibilities include conducting initial intake assessments, case management services, and aftercare planning. We believe in progressive, individualized, and evidence-based treatment. Job Duties:

  • The Clinical Intake Counselor/Case Manager assesses potential clients to determine the appropriate level of care based on ASAM criteria.
  • The Clinical Intake Counselor/Case Manager engages clients while providing support and encouragement to individuals and their loved ones. Explain programs and services offered.
  • The Clinical Intake Counselor/Case Manager provides crisis intervention services and conducts risk assessments as needed
  • The Clinical Intake Counselor/Case Manager completes biopsychosocial assessments.
  • The Clinical Intake Counselor/Case Manager reviews clinical documentation for potential clients prior to admission to identify and report any inconsistency to Outreach Specialists, Executive Directors and Clinical Directors
  • The Clinical Intake Counselor/Case Manager coordinates client information with unit staff, managers, Executive Director and Clinical Director to complete client intake.
  • The Clinical Intake Counselor/Case Manager completes all case management documentation in accordance with regulatory standards.
  • The Clinical Intake Counselor/Case Manager documents urinalysis samples and breathalyzer screenings for all CLIA approved waived testing in assistance with Nursing.
  • The Clinical Intake Counselor/Case Manager facilitates referrals to physicians, dentists and other healthcare professionals as indicated by the assessed needs of each individual patient.
  • The Clinical Intake Counselor/Case Manager coordinates necessary transportation for patients in accordance with transportation policy and procedures
  • The Clinical Intake Counselor/Case Manager conducts Vocational Assessments for clients and ensure follow through.
  • The Clinical Intake Counselor/Case Manager collaborates with individual counselors to ensure coordination of appropriate aftercare planning for each client.
  • The Clinical Intake Counselor/Case Manager facilitates the completion of discharge surveys for clients upon coordination of aftercare planning.
  • The Clinical Intake Counselor/Case Manager gathers completed admission, mid-treatment, and discharge surveys with purpose to aggregate data and compile a monthly report.
  • The Clinical Intake Counselor/Case Manager facilitates groups as scheduled by Clinical Supervisor/Clinical Director
  • The Clinical Intake Counselor/Case Manager maintains an active case management caseload providing interventions as needed and within area of expertise and limits of credential.
  • The Clinical Intake Counselor/Case Manager provides ASAM specific reviews as needed and reports findings to the Utilization Review Specialist in a timely and efficient manner.
  • The Clinical Intake Counselor/Case Manager supports units and clinical milieu by engaging in groups or individual sessions as needed
  • Other duties as assigned

Qualifications:

  • Must be able to assess for appropriate level of care with knowledge of ASAM criteria and DSM diagnostic criteria.
  • Must be able to demonstrate high level, efficient documentation skills
  • Must display professionalism and excellent customer service and de-escalation skills
  • Experience in the substance abuse or mental health field is required (1-2 years preferred)
  • Must hold current LSW or LPC
  • CADC certification is preferred

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
