Calcasieu Community Care Center is seeking a Clinical Coordinator (LPN)!

Job Responsibilities:
Provides each resident with professional nursing care, assessments and evaluations.- Responsible for the management of nursing activities performed by Neighborhood/Household (NH/HH) staff ensuring that the highest degree of quality care is maintained at all times
- Works with a team of cross-trained staff to coordinate the functions of the NH/HH
- Has 24 hour responsibility for clinical functions of the NH/HH
Qualification:
A current, valid Louisiana registered nurse (LPN) license is required- Minimum of one (1) year experience in long term care, prior MDS experience is preferred.
- Ability to effectively communicate, direct and delegate tasks.
