GENERAL DESCRIPTION SUMMARY
Under the direct supervision of a Registered Nurse, the Certified Nursing Assistant (CNA) provides a central role on the hospice and palliative care teams, not only providing personal care for the patient and assisting with activities of daily living but demonstrating competencies specific to enhancing the quality of life for patients and their families facing progressive, life-limiting illnesses. The CNA I function in a supportive relationship to professional disciplines and may function as an extension of these professional services with written instructions, appropriate supervision and specialized hospice training.
Essential Duties & Responsibilities - Within the philosophy, objectives and policies of Capital Caring Health, carries out the following essential functions:
Quality Provision of Services:
1. Demonstrates good judgment in planning and providing care to patients and families experiencing life-limiting, progressive illness to address the physical, emotional, psychosocial and spiritual needs of the patients and families.
2. Demonstrates knowledge, attitudes, behaviors, and skills consistent with the performance standards, code of ethics, and scope of practice for hospice and palliative care nursing assistants/home health aides.
3. Demonstrates cultural competence by respecting and honoring unique values, diversity, and characteristics of patients, families, and co-workers in hospice and palliative care.
4. Measures and records fluid intake and output according to order on the POC.
5. May assist patients with self-administration of medications, according to the POC.
6. Assists patients with meals, if necessary, according to the POC.
7. Assists in maintaining a safe and sanitary environment in the patient's home.
8. Assumes responsibility for professional growth and development through attendance at least quarterly in-services or training up to a minimum of 12 hours per year and recertification when appropriate.
9. Mentors other nursing assistants in the work place to share experiences, knowledge and provide support.
Organizational/Regulatory Compliance:
1. Adheres to Capital Caring Health policy and procedures, as well as compliance with all areas of federal, state and local laws and regulations, accreditation standards, Conditions of Participation and the organization’s standards of care.
2. Provides personal care, including: bathing, dressing, grooming, caring for hair, nail care, oral hygiene, skin care, and foot care, according to the Plan of Care (POC).
3. Assists with elimination, including: bed pan, urinal, up to the bathroom, routine catheter care, external colostomy and skin care according to the POC.
4. Assists patients in turning, getting up in a chair, assists patients in getting into and out of bed, including the use of mechanical lifting equipment/the use of wheelchairs and walkers, and assists patients in ambulating according to the POC.
5. Assists with prescribed exercises when the patient and the nursing assistant have been instructed by the appropriate health professional, according to the POC.
6. Takes and records temperature, pulse and respiration, as requested by the RN.
7. Observes for pain, determines pain rating using PAINAD or patient self-report and reports results to RN immediately.
8. Observes for side effects of pain medication and/or therapies and documents appropriately.
9. Reads and follows the Certified Nursing Assistant Plan of Care at each visit. Frequency of visits ordered per the POC must be individualized to meet patient needs and must match actual visits.
10. Completes appropriate documentation in a timely manner, in accordance with hospice medical records policies and procedures.
11. Participates in QUAPI and customer service programs and in-services as requested.
12. Follows infection control procedures and guidelines as outlined by the agency and regulatory bodies.
People/Communications:
1. Participates as a member of the interdisciplinary team and therefore in collaboration and coordination with the team, appropriately uses resources to enhance the quality of life for patients and families and assist them to actively address their goals.
2. Identifies and advocates for the wishes and preferences of patients and families.
3. Demonstrates effective verbal, non-verbal, and written communication with patients, families, other team members to meet the patients’ and families’ hospice and palliative goals and needs.
4. Provides spiritual care interventions which include being a good listener, encouraging religious practices and life review, respecting the patient’s and family’s beliefs and religious practices, developing a trusting relationship, and working with the team to plan and carry out spiritual care.
5. Communicates with Case Manager or Clinical Supervisor/Clinical Director to report changes in patient condition.
