Title: Case Manager – Senior Services Program

Overview
Catholic Charities, Diocese of Venice, provides many social services to all, in a large geographical region in southwest Florida.
The Senior Services Case Manager is responsible for assuring enrolled participants have all the tools they need to live independently and remain living in the least restrictive living environment. Case Mangers will provide case management to seniors (ages 55 and older) who reside in Lee County which includes conducting home visits to those that are home bound.
Job Duties
- Maintains accurate service records and documentation and prepares and submits regular reports to the Program Director per request or as required.
- Completes comprehensive assessments of potential clients through scheduled home visits; including admissions and discharge planning.
- Determines through health, emotional well-being, and nutrition assessments, a level of care that is mostly likely to prevent premature nursing home placement.
- Demonstrates knowledge of community agencies and is able to provide referrals to link clients to services as needed
- Confers with supervisor regarding client concerns and recommends methods of assisting clients of needed support and advocacy, including referrals to other agencies.
- Reviews and maintains communication about care plans and service delivery with supervisor, client and/or client caregiver.
- Follows strict confidentiality standards with respect to agency and client information in accordance with agency policy and with provisions of Health Insurance Portability and Accountability Act (HIPPA).
- Maintains record keeping an agency electronic data base.
- Other duties as assigned
- Bachelors degree in Human Service or related field
- Bilingual, English/Spanish required
- One year of work experience (internships will be included)
- Excellent communications skills both written and oral
- Strong organizational skills and attention to detail
- Ability to work with diverse backgrounds, to include the senior population and their families
- Strong computer skills including database management and Microsoft Office Suite
- Must pass a Level II background check
- Must possess a valid driver's license
