Position Summary:
As a Case Manager, it will be your responsibility to build and maintain patient relationships throughout patients' care at Sycamore Integrated Health. You will be responsible for acquiring long term, quality patients for the clinic through the implementation of proven lead conversion systems. You will participate in educating the patients while working with the clinicians as they report the findings of an orthopedic exam and diagnostic imaging. It will be your responsibility to deliver a financial breakdown of the overall Care Plan along with helping patients to coordinate their schedules to arrive for the recommended care. As a Case Manager, you will focus on understanding patient needs and wants, helping them overcome obstacles and empathizing with their struggles and challenges while influencing them to do what is in their best interest to improve their quality of life. To be a remarkable Case Manager, you must be able to develop trust and the confidence necessary to help patients navigate their Care Plan. You will need to express a breadth of knowledge and experience in these areas, as well as the ability to work and collaborate effectively within a highly team-centric environment.

PERFORMANCE OBJECTIVES
- Create and deliver financial estimates, secure financial commitment, and confirm long-term scheduling to patients based on Provider’s orders
- Develop emotional connection with patients to understand their expectations, goals, and desired outcomes
- Perform initial consultations on new patients to determine medical history, motivating factor that prompted patient to look for help with their problem and functional goals of the patient
- Discover and provide motivating factors that prompted patients to seek help.
- Establish functional goals of the patient and be able to communicate with your team.
- Execute patient follow up calls, lead conversion techniques, and patient retention activities
- Provide and ensure completion of all patient paperwork
- Effectively and continually educate patients about the need and benefits for active progress through the complete care plan
- Ensure that all of the patient’s needs are met, nothing gets overlooked, and care is consistent with the patient’s expressed desires

Education & Experience
- High school diploma or equivalent
- 2 years of Case management (preferred)
- Experience working in health care (preferred)
- Sales (preferred)

Salary Range:
$40,000 - $60,000 + Bonus
