Case Manager Associate
Hiring Bonus of $1,000 to external Candidates
Our Deaf Mental Health Services team is seeking a Case Manager Associate to work with clients who are diagnosed with chronic mental illness on our Deaf and Hard of Hearing team. This part-time position is responsible for providing a supportive environment for our clients. Responsibilities include providing person-centered engagement and interventions, medication administration, and providing general program oversight and safety.
American Sign Language (ASL) is the primary mode of communication which offers our residents a strong sense of community with other deaf adults and strengthens our support activities in a culturally affirmative environment. We provide an array of individualized services to our residents to help maintain independent living in a community setting.
Location: 726 2nd Street NE, Minneapolis, MN 55413, Hybrid with some remote work.
Hours: Tuesdays 12:00pm - 5:00pm and Fridays 12:00pm - 5:00pm, M-F: Flexible/Remote hours.
Hiring Range: $18.70 -$19.95 hourly depending on qualifications
Required Qualifications:
Required Qualifications:
- be at least 21 years of age;
- fluent in ASL
- have at least a high school diploma or its equivalent; and
- meet one of the following criteria:
- have an associate of arts degree in one of the behavioral sciences or human services;
- be a certified peer specialist under section 256B.0615;
- be a registered nurse without a bachelor's degree;
- within the previous ten years, have three years of life experience with serious and persistent mental illness as defined in subdivision 20; or as a child had severe emotional disturbance as defined in section 245.4871, subdivision 6; or have three years life experience as a primary caregiver to an adult with serious and persistent mental illness within the previous ten years;
- have 6,000 hours work experience as a nondegreed state hospital technician; or
- have at least 6,000 hours of supervised experience in the delivery of services to persons with mental illness
Preferred Qualifications:
- Experience providing direct services to adults with serious and persistent mental illness
Main Job Duties
- Responsible for running day to day duties at the drop-in center working with deaf participants.
- Coordinating community-based outings, engaging clients with events and activities.
- Using person-centered principles, actively engage with clients in their daily activities, and develop and maintain appropriate relationships that support the client in meeting their individual goals.
- Document interactions with clients into organization’s Electronic Health Record (EHR) and if applicable, program-specific documentation/record-keeping system, while providing clear and concise communication with client, staff and other applicable parties.
- Conducting outreach activities and sourcing to build engagement with potential clients.
