Job Description
- Position Title : Bilingual Service Advisor, PDS Waiver
- Division: CM Work Location: Florence Exempt Status –Non- Exempt
- Supervisor PDS Waiver Supervisor Supervises N/A
- Salary: $32,500-$35,500 Annually
Position Purpose
This is specialized and technical work in providing assessment and support broker services to PDS Waiver participants with an area of focus in bilingual case management, which includes ongoing assessment for program eligibility, monitoring all aspects of care in relation to the waiver program to ensure needs of the participant and caregivers are met. Duties are performed under moderate to limited supervision.
Responsibilities/ Duties:
Essential:
- Proficiency with the Spanish Language.
- Manage a caseload that consists of both Spanish speaking participants as well as English speaking participants and complete home visits as required and as needed to assess program eligibility, ongoing service eligibility as well as ongoing care plan for identified/continued needs.
- Responsible for eligibility assessment and care management of assigned participants within time frames and policies of the division and programs.
- Authorize appropriate services as supported by assessment and ensure services billed correctly.
- Assist participants in calculating budget based on plan of care to include appropriate taxes and administrative costs.
- Develop and update as appropriate a comprehensive plan of care with participants and caregivers, addressing the participant’s needs. Utilize all available supports (formal and informal) before authorizing NKADD administered services.
- Communicate with the participant on a variety of issues and policies related to needs, services, and resources.
- Conduct timesheet training and background checks on participants employees.
- Collect and check CDO timesheets and work with CDO FMA on payment issues.
- Communicate with providers on a regular basis when needed concerning service delivery maintenance and issues. Act as advocate for participants as needed.
- Participate in case conferences with a variety of parties to include participants, caregivers, providers, co- workers, and management.
- Participate in ongoing training required per DAIL and NKADD standards yearly.
- Maintain professional, accurate and timely documentation of all contacts and activities with participant/caregivers in approved database per policy/procedure.
- Represent program to all outside entities with professional communication.
- Maintain client physical file all required forms, documentation and other paperwork required per policy.
- Follow all confidentiality procedures, policy, and law.
- Responsible for division on call phone coverage as scheduled.
Secondary:
- Participate in agency related functions as requested such as committees and special projects.
- Provide coverage for other Brokers as needed.
- Other duties and/or projects as assigned.
Desired Minimum Qualifications
Kentucky licensed Registered Nurse or a bachelor’s degree in a Human Service Field, proficiency in Spanish language, and be in good standing with the credentialing board (if applicable) with at least one-year prior experience in medical social work and/or geriatrics and experience working with populations of all ages.
Knowledge, Skills, and Abilities:
- Ability to maintain effective relationships with co-workers and individuals from a variety of economic and cultural backgrounds. Be able to establish and maintain effective communications with all levels of professionals and participants with whom they interact from varies cultural backgrounds.
- Knowledge of the policies, laws and regulations governing confidentiality such as HIPPA.
- Ability to learn and retain information regarding multiple programs.
- Ability to read and understand regulations that apply to the program.
- Ability to accurately enter data into the computer and general computer knowledge.
- Skill in organizing and maintain accurate files and records.
- Skills in mathematics and budgeting.
- Ability to exercise judgment in the performance of duties.
- Ability to understand and follow written and oral instructions quickly and accurately.
- Ability to meet and deal tactfully and courteously with the public.
- Ability to make sound judgments.
- Good knowledge of safe work practices and ability to apply them in workday situations.
- Knowledgeable of community, state, and federal resources and how to access or utilize them.
- Be able to cope with complex and stressful client situations and variations.
- Be able to maintain and develop concepts and professional changes in assessment and case management systems or teachings.
- Ability to drive with valid license to home visits and other meetings and visits throughout the NKADD 8 county region.
- Appear for work and complete assigned tasks within a reasonable period of time.
- Become SHIP certified within 6 months of employment and maintain certification thereafter.
- Knowledge of safe work practices including those related to home visitation and protective gear as required.
Special Position Requirements
- Possess valid driver’s license and ability to operate a motor vehicle in a safe manner. This includes ability to drive and enter/exit vehicle on employees own. Position may require occasional irregular hours and district travel.
- This position requires home visits and the physical and mental ability to complete these with regard to various environments that may be encountered. This includes potential for environmental and physical hazards.
- Able to use Spanish Language fluently and accurately on all levels pertinent to professional needs.
Work Requirements
- Work is performed indoors in cubicle type office setting. It requires standing, sitting or stooping. Work requires consistent use of computer and phone system as well as other office equipment as well as mental capabilities.
- This position requires home visitations and the physical and mental ability to complete such visits.
