Outline of Essential Job Responsibilities/Functions to Include but Not Limited to:

Customer Care and Relationships:

1. Creates, develops and nurtures culturally appropriate interactions and connections with each other, customer-owners, and the community.

2. Leads, coaches, innovates, and provides input into system design in support of relationship optimization and the delivery of quality care.

3. Works in conjunction with multiple provider teams and responds to referrals requesting customer social resource needs and time-intensive care coordination tasks to include:

  • Substance Abuse and Detox Resources
  • Discharge Planning
  • Assisted Living Coordination
  • Palliative Care/End of Life Coordination
  • Community Groups
  • Community Resources
  • Medicaid/FHR
  • Waiver Care Coordination
  • Housing Coordination
  • General Relief
  • Care Conference
  • Adult Protective Services Coordination
  • Transportation Coordination
  • Personal Care Assistance

4. Applies SCF Core Concepts in their work.

5. Ensures effective communication and exceptional customer service.

6. Works with other departments for the referral process of services for customer-owner care.

7. Familiar with Medicaid programs and the four (4) waiver types and basic eligibility.

8. Proficient in applying the operational principles and the primary care model in own team as well as the ability to inspire and share with other teams and clinics.

Communications and Teamwork:

1. Shares and receives information, opinions, concerns and feedback in a supportive manner.

2. Works collaboratively by building bridges and creating rapport with team members within departments and across the organization.

3. Assures best practices are spread throughout the department and when appropriate the division:

  • Communicates regularly with clinical staff and other team members to assess for issues and opportunities.
  • Communicates regularly with supervisors, managers, and administrators about issues and opportunities to spread best practices.

4. Consistently demonstrates highly effective communication skills that optimize working relationships.

Improvement and Innovation:

1. Makes meaningful improvement to services, program, and processes and/or organizational effectiveness that creates new value for customer-owners and employees.

2. Participates in planning for improvement and innovation at the unit or department level applies knowledge of strengths, weaknesses, and anticipated challenges.

3. Consistently applies and models behavior that supports the culture of continual improvement.

4. Demonstrates knowledge of tools, methodologies, and philosophies of continual quality improvement through work on clinical practice and/or project-focused work.

5. Demonstrates knowledge of clinical quality improvement models and process.

6. Applies knowledge of quality management and improvement processes to comply with internal and external standards, i.e. Joint Commission.

7. Works with program management and senior leadership to further develop team effectiveness and innovation.

5. Proficient in the use of appropriate computer-based software and office equipment.

6. Maintains quality, safety, and infection control standards.

7. Attends management meetings as needed and acts as the subject matter expert for Waiver/Care Coordination area. Able to preliminarily screen for waiver eligibility and make appropriate referrals as needed.

8. May consult with management regarding Quality Improvement and Quality Assurance issues.

9. Participates in team training as an instructor or facilitator.

10. Develops mentoring resources for clinical and support staff.

11. Works with program management and senior leadership to further develop team effectiveness and innovation.

12. Plans and facilitates training and development of staff to improve performance.

13. Participates on various committees and represents Community Resource Specialists in department and organizational meetings as needed.

Qualifications:

SCF programs are established to serve a primary population comprised of Alaska Native people who are affiliated with Cook Inlet Region, Inc. (CIRI) and Alaska Native and American Indian people within SCF’s geographical service area. Employees should have a thorough understanding of the cultures and the needs of this population. Such knowledge is critical to ensure the achievement of SCF’s vision of a Native Community that enjoys physical, mental, emotional, and spiritual wellness, and mission of working together with the Native Community to achieve wellness through health and related services:

1. BA or BS degree in Nursing, Psychology, Rehabilitation, Social Work or a closely related Human Services field from an accredited college or university; OR equivalent combination of education, training, and experience.

Additional Qualifications for Community Resource Specialist II:

1. Master’s degree in Counseling, Education, Marriage Family Therapy, Psychology or Social Work; OR equivalent combination of education, training, and experience.
