This facility is part of The Village of St. Edward, a non-profit organization. With continued growth, we are seeking an individual who is Committed, Kind, Knowledgeable and Respectful to assume the Assisted Living Coordinator position to be part of our Excellent Wadsworth Team!

POSITION SUMMARY

In accordance with the mission and values of The Village of St. Edward, the Assisted Living Coordinator is charged, in conjunction with the Director of Nursing (AL/MC/IL), to support, supervise, and operate the Assisted Living/Memory Care. In addition, this position has the responsibility to maintain and schedule staff.

Qualifications

  • State of Ohio Licensed Practical Nurse or Registered Nurse with previous experience in Long Term Care, Assisted Living or Home Health.
  • At least 2 years of experience as a LPN/RN in long term care, assisted living or home health, more years preferred.
  • 1-2 years experience of supervisor experience preferred in Long Term Care, Assisted Living or Home Health
  • Able to lift, push, and pull up to 50 pounds independently, and over 50 pounds with assistance.
  • Possess the necessary dexterity to perform injections.
  • Possesses the ability to bend, stoop, and reach at different angles.
  • Walks, stands and sits intermittently during the day.
  • Must communicate effectively with residents, families and public on personal basis.
  • Demonstrates evidence of leadership abilities, clinical competence, and interpersonal skills.
  • Able to manage and supervise staff appropriately.
  • Effective communication including verbal, written, and presentation to varied audiences
  • Ability to manage multiple priorities and timely outcomes
  • Must have valid driver’s license and proof of auto insurance.
  • Ability to be flexible due to potential of unknowns and rapidly shifting priorities
  • Specific knowledge of Word, Excel and email or the willingness to learn. Use a range of information technology applications to fulfill the requirements of the position, produce necessary reports accurately and timely, and/or improve the functionality of the department as a whole

ESSENTIAL DUTIES AND RESPONSIBILITIES

Which must be performed in order to carry out the position purpose summarized above (general representation of the key duties and responsibilities of this position; other duties may be assigned, as required):

  • Under the direction of the Director of Nursing (AL/MC/IL) assists with overseeing all care and services provided in Assisted Living.
  • Demonstrates the ability, knowledge, and skills necessary to coordinate care services appropriate to the residents served in Assisted Living.
  • Demonstrates leadership abilities by presenting as a professional role model to staff, developing collaborative relationships, using interpersonal skills effectively including the ability to communicate, problem solve, make decisions, supervise and direct care provided by nursing assistants, and assist in orientation and education of staff.
  • Monitors work performance of all Assisted Living staff. When directing tasks requiring urgent attention, the Assisted Living Coordinator advises appropriate department director after task has been delegated. For tasks needing attention but are not urgent in nature, the Assisted Living Coordinator will advise the appropriate department director with their recommendation.
  • Responsible for compliance of current Ohio Residential Care Facility (RCF) rules and regulations.
  • Works closely with Director of Nursing (AL/MC/IL) to ensure clinical rules are met in daily operations.
  • Participates in “on-call” rotation in nursing care.
  • In conjunction with The Village of St. Edward Admissions, completes and assists with the admissions process as necessary.
  • Responsible for effective resident relations, to include handling of resident concerns and resident suggestions.
  • Schedules floor staff within the staffing guidelines and attempts to replace when a report off occurs.
  • Interviews STNA job applicants and Nurses with direction of Director of Nursing.
  • Takes report off from staff and completes required documentation.
  • Orders supplies for medical and clerical needs.
  • Completes daily nursing direct care staffing form for posting.
  • Completes data entry (including attendance points and review of timecards) into the time and attendance system timely.
  • Assists with payroll completion and prepares various payroll reports.
  • Schedules and coordinates for resident appointments within and outside of facility.
  • Attends required in-services for assisted living/memory care and facility.
  • Assists with all filing as needed and distributes copies as requested.
  • Faxes necessary documents to physicians, pharmacy as other outside providers.
  • Keeps forms current and available for use in department.
  • Completes audits on time and accurately.
  • Knowledgeable about policies, procedures and regulations.
  • Assists to prepare required paperwork for labs, discharges, admissions and transfers etc.
  • Prepares required records/papers for physicians and consultants.
  • Follows established policies of the facility.
  • Assists with Covid-19 Testing of residents and/or staff
  • Complies with the Health Insurance Portability and Accountability Act (HIPAA) which ensures that the resident’s individually identifiable health information or Protected Health Information (PHI) is kept confidential.
  • Promotes opportunities for participation in activities by all the residents according to their needs, interests and abilities, both physical and mental.
  • Assists and/or attends special functions sponsored by The Village of St. Edward.
  • Performs other duties as assigned and meets performance expectations
  • Supports the philosophy of person centered care (PCC) by actively participating in the person centered care process. Fosters development and enhancement of person centered care practices.
