Overview

Volunteers of America MN Behavioral Health Division is looking for dedicated individuals to join our team as a Mental Health Case Manager. Mental Health Targeted Case Management provides services supporting adults with mental health concerns. Managers help coordinate services to support areas such as mental health, social, education, health, vocation, recreation, volunteer, advocacy, and transportation.
VOA has provided Mental Health Case Management services over the past 21 years in Hennepin County. Over the course of this time, services were expanded to include other counties within Minnesota. This position is located in our New Hope office and serves adults in the Twin Cities area, primarily in Hennepin County.
Volunteers of America Minnesota is a nonprofit health and human services organization that annually serves more than 25,000 children, adolescents and their families, older adults, students, persons with disabilities and special needs, and adults completing incarceration. Since 1896, we have helped people gain self-reliance, dignity and hope.

Responsibilities
Job duties:
- Provide services in the four core case management components-Assessment, Service plan Development, Referral and Linkage and Monitoring, and Coordination of Services
- Coordinate with a mental health professional to assure the completion of a mental health assessment of the client's needs and to develop individual service plans.
- Develop and update the client's individualized plan (IFCSP). Coordinate, monitor, and evaluate services that are outlined in the plan for appropriateness, progress, and continued service need.
- Coordinate services with the client and other members of the multi-disciplinary team.
- Develop and utilize available area resources and make appropriate referrals.
- Meet with the client as described in the service plan and program policies.
- Keep necessary case notes documenting contact with the client and collateral contacts.
- Maintain client case files.

Requirements
- Computer proficient
- Ability to travel within Hennepin County and Metro area
- Proven skills: Prioritizing, Multi-tasking, Organized, Interpersonal skills, Networking
