JOB SUMMARY: This position is responsible for organizing activities in the facility and in the community
that are reflective of active living. These activities will meet the individual goals and interests of participants
while encouraging maximum independence. Oversee the development and implementation of active living
activities, purchasing of supplies, monthly activity calendar, planning community outings, data collection on
level of participation, developing staff flow sheets, completion of monthly reports regarding client participation
and utilization in addition to service documentation review (Alayacare).

ESSENTIAL FUNCTION/JOB DUTIES: Meet regularly with supervisor to discuss program, staff, or
participant needs and concerns. Maintain program records in accordance with Frontier Community Services
policies and procedures, the Council on Accreditation standards, program requirements, and state and federal
regulations. Develop a monthly snack menu that meets nutritional guidelines and is mailed out monthly to
participants. Develop activities with staff input that are reflective of active living, which meets the individual
goals and interests of participants, encouraging maximum independence. Develop monthly organizational plan
for assigned activities including goals and needed supplies. Purchase activity supplies and provide purchase
requests with receipts to Program Manager for monthly reconciliation. Develop a monthly activity calendar that
is mailed out to participants. Create and maintain an activity planning system that will organize activities and
supplies. Provide secondary support to the Direct Support Professionals at FMN as needed to maintain staff to
participant ratio. Submit appropriate reporting such as Critical Incident Reports or General Events Report in
accordance with State and agency policies. Attend all required trainings and meetings. Coordinate
activities/events with community agencies for participants including scheduling transportation. Provide
community education and outreach regarding adult day services. Participate in training of program staff and
volunteers. Will communicate with FMN Program Manager to procure supplies and needed maintenance in a
timely manner to assure smooth operation of the center. Other related duties as assigned.

QUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIRED: Have demonstrated skills, training,
and experience in serving the population in a manner consistent with the philosophy of the adult day program.
Must be at least 21 years of age or older and must have a degree in recreational therapy or a closely related
human services field; or two years of full-time or equivalent part-time experience in planning and leading
activities for populations similar to the recipient population. Experience working with and knowledge of older
adults and dependent care issues. Experience working with individual’s with ADRD preferred.

LICENSES AND SPECIAL REQUIREMENTS: Position is subject to a background check. Must take TB
test annually. A current Alaskan Driver License and proof of current automotive insurance.
Must have current certificates in Basic Mandt, CPR /First Aid (training provided upon hire).
