In absence of Signed Indemnity Form, instruction from non-face to face customers with the following authorization, should be notarized by a by a Notary Public, Lawyer or Bank officer with their stamp or seal affixed.

• Withdrawal from an active account
  1. Authorization Letter notarized by a Notary Public, Lawyer or Bank officer with their stamp or seal affixed.
  2. Passbook for the account
• Withdrawal from a Dormant Account
  1. Items 1 & 2 above
  2. Notarized copy of Identification – Passport / Driver’s Licence page with the photo
• Closure of an Account
  1. Items 1, 2 and 3 above
  2. In absence of the Passbook, please complete, notarize indemnity and submit our Indemnity for Lost Passbook Form
• Change of Address on an active account
  1. A notarized instruction letter containing your new address.
  2. Proof of address – utility bill, bank statement or confirmation letter from your Landlord)
